Özet
Bu çalışmanın amacı mide adenokarsinomu nedeniyle öpere edilen hastalarda tedavide prognozu etkileyen faktörleri belirlemektir. 1991-1996 yıllan arasında Ankara Numune Hastanesi 4. Cerrahi Kliniği'nde mide kanseri nedeniyle öpere edilen 87 hastada prognoza etkili olabileceği düşünülen yaş, cinsiyet, kan transfüzyonu, operasyonun acil yada elektif şartlarda yapılması, preoperatif TNM, peroperatif TNM, patolojik TNM, peroperatif Japon evrelemesi, ameliyat tipi (gastrektomi, kombine rezeksiyon, non-rezeksiyon), rezektabilite, lenfadenektomi seviyesi, operasyon ekibinin tecrübesi, tümör çapı, tümör lokalizasyonu, splenektomi, tümör invazyon derecesi, adjuvan kemoterapi, tümör differansiyasyonu ve lenf nodu metastazı univariate ve multivariate olarak analiz edildi. Univariate analiz sonucunda adjuvan kemoterapi, ameliyat tipi, rezektahilite, patolojik TNM, peroperatif TNM, peroperatif Japon evrelemesi, lenf nodu diseksiyonu, ekibin tecrübesi, tümör lokalizasyonu, tümör invazyon derecesi ve lenf nodu metastazı sağkalım üzerine etkili bağımsız değişkenler olarak ortaya çıktı. Adjuvan kemoterapi, Tümör'ün erken evrede tanınması, lenf nodu metastazının olmaması ve genişletilmiş lenf nodu diseksiyonu ile mide kanserinin tedavisinde başarılı sonuçlar elde etmek mümkün olacaktır.
Abstract
In this study, the patients that are operated for the gastric adenocarcinoma are investigated in respect of the factors affecting the prognosis and compared with the literature. Eighty seven patients were operated on because of gastric adenocarcinoma between 1991-1996 in 4th Surgical Department in Ankara Numune Hospital. The factors that probably affect the prognosis are age, sex, blood transfusion, emergent or elective surgery, preoperative TNM staging, peroperative TNM staging, pathological TNM staging, peroperative Japanese staging, type of operation performed (gastrectomy, combined resection or nonresection), resectability, level of lymphadenectomy, experience of the operation team, tumor diameter, tumor site, splenectom, tumor invasion, adjuvant chemotherapy, tumor diferentiation and lymph node metastases. These factors analysed in univariate and multivariate analyses. According to univariate analyses the type of operation performed, resectability, pathological TNM staging, pre-operative TNM staging, preoperative Japanese staging, lymph node dissection, experience of the team, tumor site, degree of tumor invasion and lymph node métastases were found to affect survival. According to mutivariate analysis; adjuvant chemotherapy, pathologic TNM staging, resectability and lymph node metastases were found to be the independent factors that affect survival. Successful results will be possible with adjuvant chemotherapy, curability, lack of lymphatic metastases and extended nodal dissection.
