Özet

Ocak 1990-Aralık 1996 arasında kliniğimizde rektum kan-seri nedeniyle ameliyat edilen 512 hastanın 51'inde lokal nüks gelişmiştir. Bunlardan incelenehilen 28 hastadaki ilk ameliyat, 7 hastada anterior rezeksiyon (AR), 16 hastada low-anterior rezeksiyon (LAR), 5 hastada abdomino-perineal rektum rezeksiyonu (APRR) dur. Lokal nükse kadar geçen süre en kısa LAR yapılanlarda en uzun APRR ya-pılanlarda bulunmuştur. Nükste en sık görülen bulgu sfinkter koruyucu ameliyat yapılanlarda kabızlık, tenesmus ve rektal kanama, APRR yapılanlarda ise sakral ve perineal ağrı ile kitledir. Nüks gelişen ve sfinkter koruyucu ameliyat yapılan 23 hastanın sadece 10'unda APRR yapılabilmiş, diğerlerinde palyatif işlemler, 2'sinde sadece biyopsi yapılabilmiştir. APRR yapılmış 5 hastada ise radikal hiç bir işlem yapılamamıştır. Radyo-kemoterapinin surviye ve semptomlara pek etkisinin olmadığı görülmüştür.

Abstract

Local recurrence has been observed in 51 of 512 patients operated for rectum Ca between January 90 and December 96 in our clinic. In 28 of these patients, primary operations were anterior resection (AR) in 7 patients, low-anterior resection (LAR) in 16 patients and abdomino-perineal rectum resection (APRR) in 5 patients, Period of local recurrence was shortest in patients performed LAR and logest in patients performed APRR. Most common findings in recurrence patients with operations of sphincter-saving type were constipation, tenesmus and rectal bleeding, whereas patients performed APRR showed symptoms of sacral or perineal pain and mass. In 23 pa-tients with local recurrence after sphincter-saving type of operations, APRR could be performed in only 10 patients. In the rest of the patients paliative procedures were performed and in 2 of the patients only biopsy could be possible. In 5 patients performed APRR no radical procedure could be done. It was shown that radio-chemotherapy has no effect on either survival time or symptoms.
