Özet

Medikal tedavi semptomatik iyileşme sağlasa da Crohn'lu hastaların birçoğunda cerrahi tedaviye gereksinim doğmaktadır. Rezeksiyon, by-pass, ve striktüroplastiler Crohn'da cerrahi seçeneklerdir. 1994-1998 yıllan arasında İ.Ü. Cerrahpaşa Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda Crohn'lu 17 hastaya 18 ameliyat yapılmıştır. Hastalarımızın 8'i erkek, 9'u kadın yaş ortalaması 34.9'du. Karın ağrısı (% 82.3), diyare (% 58.8) kilo kaybı (% 29.4), iştahsızlık (% 29.4) rektal kanama (% 17.6), enterokütan fistül (% 5.8) bağlıca şikayetlerdi. Üç hastada analfissür de saptandı. Üç hastada poliartrit, bir hastada eritema nodosum şeklinde ekstraintestinal Crohn bulguları da vardı. 15 ameliyat elektif, 3 ameliyat acil olarak gerçekleştirildi 11 olguda terminal ileum rezeksiyonu ve sağ hemikolektomi, l olguda segmenter jejunum rezeksiyonu, 2 olguda subtotal kolektomi ve ileostomi, l olguda kompletisyon kolektomisi, l olguda low anterior rezeksi-yon, 2 olguda terminal ileum ve çekum rezeksiyonu uygulandı. Serimizde mortalité gözlenmedi. 2 olguda gelişen enterokütan fistül tıbbi tedavi ile iyileşti. Postoperatif hospitalizasyon 23.7 (11-45) gündür. Crohn'da cerrahi tedavi komplikasyonlar gelişmeden elektif olarak yapılmalıdır. Nüks olasılığının yüksek ve karsinom olasılığının yüksek oluşu gibi nedenlerden dolayı bypass yerine rezek-siyon veya endikasyonları olduğunda striktüroplastiler tercih edilmelidir.

Abstract

Although medical treatment provides symptomatic relief, in the majority of patients with Crohn's disease, surgical interventions become necessary. Resection, by-pass procedures, and stricturoplasties are the surgical options in Crohn's disease. Between 1994-1998, 17 patients with Crohn's disease underwent 18 operations at the department of General Surgery in Cerrahpaşa Medical School of Istanbul University. 8 patients were men, and 9 were women, their mean age was 34.9. Abdominal pain (% 82.3), diarrhea (% 58.8), loss of weight (% 29.4), loss of appetite (% 29.4), rectal bleeding (% 17.6), enterocutane-ous fistulas (% 5.8) were the main complaints at admission. Three of them have hadfissure-in-ani when their disease has been diagnosed. Extraintestinal Crohn's disease has been manifested as poliarthritis in three cases and as erythtema nodosum in one. 15 operations were performed in elective conditions, and three were performed at emergency unit. In 11 cases, right hemicolectomy which was extended to distal ileum, in one case segmentar resection of jejunum, in two cases subtotal colectomy and ileostomy, in one case completion colectomy, in one case low anterior resection, in two cases distal ileum and caecum resection were performed. No mortality was observed. Enteroc-utoneus fistulas occurred in two cases and they were treated by medical therapy. The average postoperative hospitalization was 23.7 days (11-45). In Crohn's disease, surgical interventions should be undergone before the disease is complicated. Due to the high incidence of recurrence and malignant degeneration, it is advisable to prefer resections, or stricturoplasties whenever possible instead of by-pass procedures.
