Özet
İleri cerrahi ve onkolojik tedavi yöntemlerine rağmen mide kanserinin mortalitesi yüksektir. Ancak erken mide kanserinde 5 yıllık yaşam oranı % 95'lere çıkmaktadır. Erken mide kanserleri Japonya'da % 50'lere varan oranlarda görülmesine karşılık batı ülkelerinde bu oran % 10-15 civarındadır. GUTF Patoloji Anabilim dalında 1991-1997 yılları arasında tümör nedeni ile opere edilen 67 adet gastrik rezeksiyon materyali incelenmiştir. 67 vakanın 56'sı (% 83.5) ileri evre kanser, l J'i (% 16.5) erken mide kanseriydi. Bu tümörlerin % 63.6'sı antrum da lokalize idi. Vakaların (4 vaka) % 36.3'ün de tümör birden fazla odakta izlendi. Makroskopik olarak iki vaka tip IIc diğerleri kombine tiplerdi. 11 erken mide kanserinin 6'sı (% 54.5) mukozal, 5'i (% 45.4) submukozal lokalizasyonluydu. Histopatolojik olarak % 36.3'ü tubuler adenokanser, % 18.1'i papiller adenokanser, % 36.3'ü taşlı yüzük hücreli kanserdi. 11 vakanın 8'ine lenf nodu disseksiyonu yapılmıştı. 8 vakanın, 4'ü submukozal, 4'ü mukozal olup hiçbir vakada lenf nodu metastazına rastlanmadı. Erken mide kanseri oranları bizim serimizde de batı ülkeleri ile benzerlik göstererek % 16.5'tür. Ülkemizde de mide kanserinin sık görülen bir tümör olması nedeni ile endoskopik ve radyolojik yöntemlerin tarama programı çerçevesinde daha geniş kitlelere yaygınlaştırılması gerekliliği ve böylece tümörlerin erken evrede görülme oranlarının artacağı düşünülmektedir.
Abstract
Despite the advanced surgical and oncologic treatment modalities gastric cancer mortality remains high. But 5 years survival rate for early gastric cancer is as high as 95 %. Early gastric carcinoma accounts 50 % of gastric cancers in Japan but only 10-15 % in western countries. 67 gastric resection materials which were operated for gastric tumor were examined at Gazi University Medical School Department of Pathology between 1991-1997. Among these 67 gastric resection materials 56 (83.5 %) were advanced cancer and 11 (16.5 %) were early gastric cancer. 63.6 % of early gastric tumors were localized in the antrum. In 36.3 % of the materials tumor were multicentric. Macroscopically two of them were types lie, all the others were combined type. Tumors were confined to mucosa in 6 specimens (54.5 %), and in submucosa in 5 specimens (45.4 %). Histopathologically 36.3 % were tubuler adenocancer, 18.1 % were papiller adenocancer, 36.3 % were signet cell carcinoma. Lymph node dissection was performed in 8 of 11 patients and there were no lenf node metastasis. Among those 8 specimens, 4 were confined to mucosa, the other 4 were confined to submucosa. In our study early gastric carcinoma rate is 16.5 %, which is similar to the western countries. As gastric cancer is a common type of cancer in our country, if we perform en-doscopie and radiologie methods as survey analysis the incidence of early gastric carcinoma would be higher.
