Özet
Hidatik kist hastalığı, ülkemizde bir halk sağlığı sorunudur. Ekinokpkkus granulozus, bölgemizde en sık saptanan hidatik hastalık etkenidir. Medikal'tedayideki gelişme- 1ère rağmen; hastalığın esas tedavisi cerrahidir. Ocak 1985-Aralık 1995 tarihleri arasında kliniğimizde tedavi edilen 138 abdominal hidatidozlu olgu, bu makalede retrospektif olarak değerlendirilmiştir. Doksanüç (% 67.3) olgu kadın, 45 (% 37.7) olgu erkek olup, yaş ortalaması 40.9 (15-85 arası) idi. Hastalık 118 olguda karaciğerde, 21 olguda karaciğer dışı yerleşimli idi. Abdominal ağrı, abdominal kitle, ateş, biliyer kolik ve sanlık en sık rastlanan semptomlar idi. Tanıda en sık abdominal ultraso-nografi ve bilgisayarlı batın tomografisi kullanılmıştır. En sık uygulanan ameliyatlar, 63 (% 45.6) olguda eksternal kapitonaj+drenaj ve 28 (% 20.3) olguda parsiyel kistekto-midir. Kistektomi ve omentoplasti ameliyatları ile karşılaştırıldığında, eksternal kapitonaj+drenaj ameliyatı uygulanan olgularda, ameliyatın postoperatif komplikasyon gelişimi ve hastanede kalış süresine etkisi istatistiksel olarak anlamlı bulunmuştur (p
Abstract
SUMMARY Abdominal Hydatidpsis Hydatid disease is still a public health problem in Turkey. Echinococcus Granulosis is the most common cause of the disease in our region. Despite the improvements of medical therapy, surgeiy is still the principal therapy of the disease. From January 1985 to December 1995, 138 consecutive cases with abdominal hydatidosis were treated by operations at Department of Surgery, School of Medicine, University of Çukurova. Of these, 118 had liver hydatidosis, the others were located outside of the. liver. There were 93 (67.3 %) females, 45 (37.7 %) males, median 'age was 40.9 years. Abdominal pain, abdominal mass, fever, biliary colic and jaundice were the major symptoms of the cases at presentation. Diagnosis was based on abdominal ultrasound and abdominal CT. External capitonnage and partial. cystectomy+ external capitonnage were mostly performed. The complication rate was 16.6 %. Follow-up period was 24.7 months, and 14 (10 %) of the cases reop-erated. Pneumothorax and subdiaphragmatic abscess were the most common complications. Median hospital stay was 20.3 days.
