İkinci ve üçüncü derece internal hemoroidlerin tedavisinde lastik bant ligasyonu ile hemoroidektominin karşılaştırılması

Özet
Aralık 1994-Eylül 1997 tarihleri arasında polikliniğimize anorektal şikayetlerle başvuran olgulardan 2. ve 3. derece internal hemoroid saptanan ve başka hiçbir anorektal patolojisi olmayan 103 olgunun 62'sine lastik bant ligasyonu (LBL), 41'ine ise Modifıye Milligan-Morgan tekniğiyle hemoroidektomi (MMMH) uygulandı. 8 haftalık takip sonrasında LBL uygulanan olgularda MMMH'ye göre müdahale sonrası dönemin daha rahat olduğu, hastanede yatışın özel haller dışında gerekmediği, işgücü kaybının ve komplikasyonların daha az görüldüğü, müdahaleyi başarılı bulan ve aynı yöntemi yine tercih edebileceğini ifade eden olguların çoğunluğu oluşturdukları saptandı. Sonuç olarak, LBL'nin 2. ve 3. derece internal hemoroidlerin tedavisinde MMMH'ye iyi bir alternatif olduğu ve tedavide ilk olarak seçilmesinin uygun olacağı kanaatine varıldı.
Abstract
The subjects of this study consists of 103 cases which were chosen among the ones who came to our clinic between December 1994-September 1997 with anorectal complaints and whom were diagnosed 2nd and 3rd stage of internal hemorrhoids without any other anorectal problems. Rubber band ligation (RBL) was performed to 62 of 103 cases and Modified Milligan Morgan's Hemorrhoidectomy (MMMH) was applied to remaining 41 cases. After 8 weeks of follow up, the cases whom were applied RBL have had prompt symptomatic relief, could start working in a very short time, had no need hospitalization expect special conditions and had lower complications than MMMH group. In addition, the number of cases who found the intervention successful and who said to prefer the same method incase their complications reoccur were higher in the RBL group than MMMH group. As an outcome; it is suggested that RBL should be considered as a good alternative to MMMH in the treatment of 2nd and 3rd degree of internal hemorrhoids even for recurrences and MMMH should be planned for the cases only when RBL fails.
