Özet

Primer retroperitoneal malign tümörler nadir görülen, nüksleri oldukça sık ve mortalitesi yüksek tümörlerdir. Bu çalışmada primer retroperitoneal malign tümör nedeniyle opere edilen dokuz olgunun sonuçlarının histopatolojik tanı, uygulanan cerrahi yöntem, perioperatif mortalite, nüks ve survi açısından irdelenmesi amaçlandı. Genel Cerrahi Servisine 1994-1999 yılları arasında müracaat eden dokuz primer retroperitoneal malign tümörlü hastaya cerrahi girişim uygulandı. Olguların beşine komplet rezeksiyon uygulanırken, dört hastaya ise tümörün vital yapılara invazyon göstermesi nedeniyle inkomplet rezeksiyon uygulandı. Histopatolojik inceleme sonucu iki olguda nöroblastoma, iki olguda malign schwannoma saptanırken, birer olguda kondrosarkom, hemanjioperisitoma, leiomyosarkom, paraganglioma ve hemanjioendotelioma saptandı. Komplet rezeksiyon uygulanan bir hasta ile inkomplet rezeksiyon uygulanan diğer bir hasta perioperatif dönemde kaybedildi. İnkomplet rezeksiyon uygulanan diğer üç hasta ilk 16 ay içinde kaybedildi. Komplet rezeksiyon uygulanan bir olguda dört yıllık takipte, bir diğerinde ise iki yıllık takipte nüks saptanmadı. Komplet rezeksiyon uygulanan diğer iki vakada bir yıllık takipte nükse rastlanmadı. Retroperitoneal malign tümörlerin invazyon yaptığı komşu dokularla birlikte komplet olarak rezeke edilmesinin surviyi olumlu yönde etkilediği kanısına varıldı.

Abstract

Primary retroperitoneal malignant tumors, which have high recurrence and mortality rates are very rare diseases. The aim of this study is to evaluate the results of histopathologic diagnosis, surgical procedure, perioperative mortality, recurrence, and survival in nine patients who were operated for primary malignant retroperitoneal tumors. Nine patients who were operated for primary retroperitoneal malignancy between 1994 and 1999 in Department of General Surgery were included in the study. While complete resection was applied to five patients, the other four patients underwent incomplete resection because of tumoral invasion vital structures. Histopathologic examination revealed that the resected tumors were malignant schwannoma (n=2), neuroblastoma (n=2), leiomyosarcoma (n=1), hemangiopericytoma (n=1), chondrosarcoma (n=1), hemangioendothelioma (n=1) and paraganglioma (n=1). One patient who had complete resection and the other with incomplete resection, were died in the perioperative period. The other three patients who underwent incomplete resection were died within 16 months. Two patient with complete resection had no recurrence within 4-year and 2-year follow-up, respectively. The other two patients with complete resection had no recurrences within one year follow-up. It is concluded that complete resection of the primary malignant retroperitoneal tumors with surrounding structures invaded by the tumor may prolong survival.
