Özet
Meme hastalıkları ile uğraşan hekimlerin en sık karşılaşmış oldukları ve kadınların % 70'ini etkileyen mastalji, yeterince anlaşılmayan bir problemdir. Nisan-1996 ile Kasım-1997 tarihleri arasında, meme ağrısı nedeniyle başvuran 60 olgu, prospektif olarak uygulanan protokole göre değerlendirildi. İzlenme süresi, en az 3 en çok 20 ay olmak üzere, ortalama 15 ay idi. Protokol için kullanılan kriterler, etyolojik faktörler, anamnez, fizik muayene bulguları, modifiye Tursky ve lineer ağrı skalası yardımıyla ağrı özellikleri, laboratuvar ve radyolojik bulgular idi. Basit analjezikler, birinci basamak tedavide tercih edilirken, ikinci basamakta hormonal maniplasyonların etkinliği değerlendirildi. Çalışmaya, göğüste kas iskelet sistemi ağrısı veya malign lezyonu olduğu düşünülen hastalar, dahil edilmedi. Tedavi protokolüne uymayan 2 (% 3.3) hasta dahil edilmedi. Memeyi sıkıca saran sutyen kullanılması önerisi, ağrı özellikleri hakkında bilgi verilmesi ve basit analjeziklerin kullanımı, birinci basamak tedavi idi ve olguların % 83.3'ü bu yolla tedavi edildi. Hastaların % 16.7'sinde, hormonal ilaçlara gerek duyuldu. Bu olguların, % 80'inde prolaktin inhibitörü olan Lisurid kullanıldı. Geri kalan hastalara, diğer hormonal ilaçlar gerekli oldu. Abstract
Mastalgia, that physicians relevant with breast disease have encountered the most frequently and affected 70% of woman, is adequately incomprehensible problem. Sixty patients admitted with mastalgia to the Dicle University Medical Faculty General Surgery Department between April 1996 and November 1997 were evaluated according to the protocol applied prospectively. Overall follow-up period was 15 months, ranged between 3 and 20 months. The criteria used for the protocol were etiologic factors, history, physical examination, features of the pain defined linear, modified Tursky pain descriptors scales, laboratory and radiological findings. Simple analgesic drugs were preferred as a first step treatment and effects of hormonal manipulations evaluated on the second step treatment. The patients thought to have malign lesions or muscle-skeletal pain of the chest were not included the study. Two patients(3.3%) who did not compatible the protocol of the treatment were also omitted. To give information about the features of the pain and suggest the use of clothes surrounding the breast firmly and simple analgesic drugs were the first step management, 83.3 % of the patients were treated by that way. Only in 16.7 % of the patients usage of hormonal preparations was needed. Lisurid -on inhibitory factor of prolactin- was used In 80% of those patients. In the rest of the patients , other hormonally drugs were required. Statistical significance was found between the groups for etiological factors of mastalgia, which are age, following period, menarche age and first pregnancy age. In conclusion, we thought that if the patients with mastalgia evaluated well and followed, only 17% of them needed further laboratory investigations and hormonal manipulations.
