Kolesisto-enterik fistül ve safra taşı ileusu

Özet
Bu çalışmada kliniğimizde Ocak 1992-Mart 1999 tarihleri arasında kolesisto-enterik fistül (KEF) ve safra taşına ileusuna bağlı akut mekanik intestinal obstrüksiyon tanısı ile ameliyat edilen 4 hastanın verileri retrospektif olarak incelendi. Olguların üçü kadın (61, 64 ve 69 yaşlarında), diğeri ise erkek (56 yağında) idi. Anamnezlerinde, 3 olgu daha önceden kolelitiazise ait semptomlar tanımlarken bir olgunun bu açıdan öyküsünde bir özellik yoktu. Olguların tamamında KEF safra kesesi ile duodenum arasındaydı. Başka bir sağlık kurumunda kolesistektomi ve duodenuma primer onarım uygulanan bir olgu ile direkt olarak bize başvuran ve genel durumu kötü olması ve taşın bulunduğu lokalizasyonda ince barsak iskemisi olması nedeni ile bir olguya sadece enterolitotomi+uç uca anastomozu takiben ameliyat sonlandırıldı. Diğer iki olguya aynı seansta hem enterolitotomi hemde kolesistektomi ve duodenumun primer onarımı (olgulardan birinde ileumda nekroz nedeni ile ince barsak rezeksiyonu+uç uca anastomoz yapıldı) uygulandı. İki olguda keşi yeri enfeksiyonu gelişmesi dışında başka bir komplikasyon olmadı. KEF ve safra taşı ileusunda, hastanın genel kondisyonu ise aynı seansta kolesistektomi, fistülün onarımı ve taşın çıkarılması, ancak hastanın genel durumu iyi değilse ilk seansta yalnızca enterolitotominin yeterli olacağı kanısına varıldı.
Abstract
A retrospective analysis of four patients, operated between October 1992-March 1999 with the diagnosis of mechanical intestinal obstruction, caused by cholecysto-enteric fistula and gallstone ileus was made. Three of the patients were women (61, 64 and 69 years old), fourth patient was 56 year old age and man. Previous biliary symptoms had been observed in three of the cases. The fistula had been localized between gallbladder and duodenum in all of the cases. In one of the cases, cholecystectomy and repair of the duodenum had been applied, in another hospital. For one patient we didn't perform the fistula repair, because the general condition of the patient was poor. In the other two cases, we performed in addition to enterolithotomy, a resection of the fistula as one-stage repair. There were no complication for four except wound infection in two patients at the postoperative period. In conclusion; if the general condition of the patient is convenient cholecystectomy, enterolithotomy and repair of the fistula can be applied in most patients with cholecystoenteric fistula and gallstone ileus, but if the general condition of the patient is poor enterolithotomy alone will be enough as the first session.
