Özet

Bu çalışmada tiroid nodüllerinin malinite açısından değerlendirilmesinde preoperatif ince iğne aspirasyon biyopsisi (İİAB) ve intraoperatif frozen section'in (FS) etkinlikleri araştırıldı. Retrospektif olarak Eylül 1994-Eylül 1999 tarihleri arasında Yüzüncü Yıl Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda tiroid nodülü nedeniyle opere edilen 196 hastanın dosya bilgileri incelenerek İİAB, FS ve son histopatolojik tanıları değerlendirildi. Malin ve şüpheli olgular pozitif malinite, benin ve foliküler lezyonlar negatif malinite kabul edilerek hesaplanan İİAB'nin sensitivitesi % 83, spesifisitesi % 93 ve doğruluk oram % 92 olarak bulunurken; FS'nin sensitivitesi % 81, spesifisitesi % 97 ve doğruluk oram % 94 olarak bulundu. Şüpheli lezyonlar çalışma dışı bırakıldığında İİAB'nin sensitivitesi % 75, spesifisitesi % 100 ve doğruluk oranı % 98; FS'nin sensitivitesi % 80, spesifisitesi % 100 ve doğruluk oranı % 97 olarak bulundu. İİAB tekniğine uygun olarak yapılıp ve deneyimli bir patolog tarafından değerlendirildiğinde, tiroid nodüllerinin malinite açışından değerlendirilmesinde etkin bir tanı yöntemidir. İİAB sonucu şüpheli ve negatif olan fakat klinik olarak malinite şüphesi olan vakalarda intraoperatif FS doğru tanı olasılığını artırır.

Abstract

The purpose of this study was to research the effectiveness of Fine Needle Aspiration Biopsy (FNAB) and intraoperatively performed Frozen Section (FS) in the evaluation of thyroid nodules for malignancy. 196 patients who were operated for thyroid nodules at General Surgery Department of Yüzüncü Yıl University Medical Faculty between September 94 and September 99 were evaluated for FNAB, FS and final histopathological diagnosis retrospectively. Sensitivity, specificity, and confirmation rates of FNAB, which were calculated as 83 %, 93 %, 92 %, and sensitivity, specifity and confirmation rate of F S were found 81 %, 97 %, 94 % respectively when malignant and suspicious cases were accepted as a positive malignancy and benign and folicular lesions were accepted as a negative malignancy for thyroid nodules. When suspicious lesions were excluded sensitivity, specificity and confirmation rate of FNAB were found as 75 %, 100 %, and 98 %, and sensitivity, specificity and confirmation rate of FS were found as 80 %,100%, 97% respectively. As a result, in the evolution of thyroid nodules for malignancy, FNAB is one of the best diagnostic methods when performed in correct technique and examined by an experienced pathologist. If the result of FNAB is malignant for thyroid nodule, there is no need for FS examination. In those patients with a suspicious or negative result with FNAB, and clinical suspicious of malignancy, intraoperative FS examination may increase the diagnostic accuracy.
