Özet

Akut apandisit tanısı modern cerrahi uygulamada halen zorluklar arzetmekte ve en önemli tanı problemlerinden biri olmaya devam etmektedir. Bu prospektif klinik çalışmada, lökosit sayısı, C-reaktif protein (CRP), amilaz, kreatin fosfokinaz (CPK), laktik dehidrogenaz (LDH), ve fibrinojen gibi biyokimyasal parametreler kullanılarak elde edilen sonuçların akut apandisit tanı doğruluğu ile olan ilişkilerinin incelenmesi amaçlandı. Çalışmaya akut apandisit öntanısı ile hospitalize edilen 100 ardışık hasta dahil edildi. Tüm hastalardan lökosit sayısı, CRP, amilaz, CPK, LDH ve fibrinojen tayinleri için kan örneği alındı. Operasyon sonrası elde edilen histopatolojik sonuçlara göre hastalar perfore apandisit (PA), süpüratif apandisit (SA), inflame apandisit (İA) ve normal appendiks (NA) olarak sınıflandırıldı. Gözlem süreleri sonunda opere edilmeksizin taburcu edilen hastalar NA grubu ile birlikte değerlendirildi. Perfore ve süpüratif apandisitli olguların hepsinde CRP değerleri pozitif olarak saptanırken, normal appendiks saptanan olguların hiçbirinde CRP değerleri pozitif değildi. Inflame apandisit olgularındaki CRP pozitifliği % 64.5 idi. P A, SA ve İA gruplarında saptanan CRP değerleri NA grubuna göre anlamlı olarak yüksek saptandı. CRP'nin duyarlılığı % 83.6, özgünlüğü % 100, pozitif prediktif değeri % 100, negatif prediktif değeri % 69 ve doğruluğu % 88 olarak saptandı. Ortalama lökosit sayısı en yüksek SA grubunda izlenirken, bunu PA, IA ve NA grupları izledi. Apandisit saptanan tüm gruplarda lökosit değerleri apandisit saptanmayan olgulara göre anlamlı olarak yüksek idi. Lökosit sayısının duyarlılığı % 88.2, özgünlüğü % 62.5, pozitif prediktif değeri % 88.2, negatif prediktif değeri % 62.5 ve doğruluğu % 82 olarak tespit edildi. Serum amilaz, CPK, LDH ve fibrinojen sonuçları ile histopatolojik veriler arasında pozitif veya negatif korelasyon tespit edilmedi. CRP ve lökosit değerlerinin akut apandisit tanısında güvenilir belirteçler olduğu ve bu iki parametrenin birlikte değerlendirilmesi ile negatif appendektomi ve buna bağlı morbiditenin önemli oranda azalabileceği kanısına varıldı.

Abstract

Diagnosis of acute appendicitis still has difficulties in modern surgical practice and continues to be one of the most important diagnostic problems. The aim of this prospective study is to investigate the relationship between diagnostic accuracy of acute appendicitis and various biochemical parameters such as leukocyte count, C-reactive protein (CRP), serum amylase, creatine phosphokinase (CPK), lactic dehydrogenase (LDH) and fibrinogen. One hundred consecutive patients who were hospitalised with tentative diagnosis of acute appendicitis were included in the study. Blood samples were withdrawn from all patients for biochemical analysis. Patients were separated into perforated appendicitis (PA), suppurative appendicitis (SA), inflamed appendicitis (IA) and normal appendix according to their postoperative histopathological results. Patients who were discharged following observation period without an operation were evaluated with NA group. While all CRP results were positive in PA and SA groups, they were all negative in N A group. CRP was found positively in 64.5 % of patients with I A. CRP values in PA, S A and IA groups were significantly higher than that of NA group. Sensitivity, specificity, negative predictive value, positive predictive value and accuracy of CRP for diagnosis of acute appendicitis were 83.6%, 100%, 100%, 69% and 88%, respectively. While mean leukocyte count was the highest in SA group, it decreased in PA, IA and NA groups, in order of frequency. Leukocyte counts in all groups with appendicitis were significantly higher than that of without appendicitis. Sensitivity, specificity, negative predictive value, positive predictive value and accuracy of leukocyte counts were 88.2%, 62.5 %, 88.2%, 62.5% and 82%, respectively. No positive or negative correlation was detected between serum amylase, CPK, LDH, fibrinogen values and histopathological results. It is concluded that both CRP and leukocyte count are reliable indicators for accurate diagnosis of acute appendicitis and assessment of these two parameters together may decrease negative appendectomy rate and related morbidity.
