Karaciğer iskemi reperfüzyon hasarında hiperbarik oksijen tedavisi ve pentoksifilin'in etkileri (Deneysel çalışma)

Özet
Oluşturulan hepatik iskemi reperfüzyon modelinde, HBO ve pentoksifilin etkileri araştırıldı. 3 grup tavşanda yapılan çalışmada; birinci gruba / saat iskemi ve 2 saat reperfüzyon uygulandı. İkinci gruba ilave olarak 2 saatlik reperfüzyon süresince 2.4 ATA basıncında ve 50 feet derinliğinde HBO uygulandı. Üçüncü gruba ilave olarak reperfüzyon öncesi 40 mg/kg dozda pentoksifilin uygulanıp, sonra 2 saat HBO ve reperfüzyon uygulandı. Çalışma sonunda tüm deneklerden kan ve karaciğer dokusu alındı. Kan örneklerinden AST, ALT, LDH, Alkali fosfataz ve NO düzeyleri incelendi. Karaciğer dokusundan hazırlanan kesitlerde de hepatosit dejenerasyonu, Kuppfer hücrelerinde artış, sinusoidal alanlarda iltihabı hücre infiltrasyonu, fokal nekroz, sinusoidlerde dilatasyon derecesi ve portal alanlarda inflamasyon varlığı araştırıldı. Sonuç olarak pentoksifilinin karaciğer iskemi reperfüzyon hasarının önlenmesinde olumlu etkilerinin olmasına karşın HBO'nün bu hasarı şiddetlendirdiği sonucuna varıldı.
Abstract
The effects of HBO and pentoxifylline are investigated on the hepatic ischemia reperfusion model. Study was performed on three groups of rabbits. Ischemia for 1 hour and reperfusion for 2 hours were applied to the first group. Additional HBO therapy at 2.4 ATA pressure and 50 feet depth was performed to the second group during the reperfusion phase. In the third group 40 mg/kg pentoxifylline was administered before reperfusion and then reperfusion with HBO was performed. Blood and hepatic samples were taken from all of the rabbits at the end of the study. AST, ALT, LDH, alkaline phosphatase and NO levels were evaluated in the blood samples. Hepatocyte degeneration, increase in the number of Kuppfer cells, inflammatory cell infiltration around sinusoids, focal necrosis, levels of sinusoidal dilatation and presence of portal inflammation were investigated in the hepatic samples. It is concluded that although pentoxifylline has positive effects on prevention of hepatic ischemia reperfusion injury, HBO therapy intensifies this injury.
