Özet

Majör cerrahi operasyon geçiren hastalarda erken enteral beslenmenin tolerabilitesinin ve komplikasyonlarının irdelenmesi amaçlandı. Ocak 1997-Ocak 2001 tarihleri arasında kliniğimizde majör cerrahi operasyon geçiren ve erken enteral beslenme uygulanan 22 olgu retrospektif olarak incelendi. Hastalarımızın yaş ortalaması 65 (43-81) idi. Olgulardan 1.4'ii erkek, 8'i kadındı. Mide kanserli 11 olguya total ve subtotal gastrektomi, özofagus kanserli 6 olguya total özofajektomi uygulandı. Bir olguya ampulla vateri tümörü, üç olguya pankreas başı kanseri, bir olguya da duodenum tümörü nedeniyle Whipple operasyonu yapıldı. Tüm hastalara Witzel jejunostomi uygulandı. Yine tüm hastalara postoperatif 6. saatten itibaren jejunostomi kalelerinden pompa seti ile 16 saat enteral beslenme yapılıp 8 saat ara verildi. Postoperatif l. günde saatte 25 mL, 2. gün saatte 50 mL, daha sonraki günlerde saatte 75 mL ile enteral beslenmeye devam edildi. Hastalar normal gıda alımına geçinceye kadar aynı dozda enteral beslenme sürdü. Ortalama normal oral gıdaya geçiş süresi 6.2 gündü. Üç olguda distansiyon, 10 olguda hafif şiddette diare gelişti. Ortalama yatış süresi 9,28 gün idi. Mide kanseri nedeniyle subtotal gastrektomi yapılan bir olguya anastomoz kaçağı nedeniyle relaparotomi uygulandı. Hastalarımızda erken dönemde mortalite görülmedi. Enteral beslenmenin bakteriyel translokasyonu önlediği ve septik komplikasyonları azalttığı deneysel ve klinik olarak gösterilmiştir. Biz de bu çalışma sonucunda erken enteral beslenmenin oldukça iyi talere edildiğini ye herhangi önemli bir komplikasyona yol açmadığını düşünüyoruz.

Abstract

Current study was planned to investigate the tolerability and complications of early enteral nutrition in patients undergoing major abdominal surgery. Between January 7997 and January 2001, early enteral nutrition was applied to twenty-two patients with major abdominal surgery and they were evaluated retrospective-ly. The mean age of the patients was 65 (between 43 and 81). Of them 14 were male and 8 were female Total and subtotal gastrectomies in eleven patients with gastric cancer and total esophagectomies in six patients with esophageal cancer were performed. Whipple procedure was performed for five patients; three with pancreatic head cancer, one with papilla vateri cancer and one with duodenal cancer. Witzel jejunostomy was performed to all patients. All patients received enteral nutrition for 16 hours by feeding pump beginning just af ter the sixth postoperative hour. Feeding was applied 25 mL per hour for the first day, 50 mL per hour for the second day and 75 mL per hour on the following days. Oral feeding started on the sixth day. We have observed abdominal distention in three, and mild diarrhea in ten patients. The mean hospital stay was 9,28 days. One patient underwent relaparotomy for anastomotic leakage. No mortality was observed in the short period Enteral nutrition was approved for preventing bacterial translocation and decreasing septic complications in clin-ical and experimental studies. We think that enteral nutri-tion is well tolerated and cause no major complications.
