Özet

Özofagus normal epitelinin yerinde Barrett epiteli gelişimi hemen hemen daima uzun süreli şiddetli gastro-özofageal reflü ile birliktedir. Reflü materyalindeki asit, safra veya duodenal içerikten hangisi ya da hangilerinin bu durumun patofizyolojisindeki yeri halen netlik kazanmamıştır. Bu çalışmada, evre I ve evre III reflü özofajitte proton pompa inhibitörleri (omeprazol veya lansoprazol) ve bir prokinetik ajan (sisaprid) ile tedavinin kolumnar epitel gelişimini önleyici etkisini ve cerrahi uygulanan olgularla sonuçlan değerlendirildi. 1993-1999 yılları arasında Savary evre II ve III reflü özofajit bulunan 81 hasta çalışmaya dahil edilmiştir. Bütün hastalardaki teşhis yöntemi endoskopi ve biyopsi idi. Elliiki hastaya 24 saatlik ambulatuar pH-metre uygulanmıştır. İlk 40 hastaya omeprazol 40 mg ve sisaprid 40 mg, sonraki 41 hastaya lansoprazol 60 mg ve sisaprid 40 mg verilmiştir. Tedavi süresi 6-22 ay (ortalama 13 ay) dır. Onaltı hasta farklı zamanlarla medikal tedaviyi takiben anti-reflü prosedürü uygulanmak üzere ameliyat edilmiştir. Hasta takipleri endoskopi ile ve en az dört kadrandan biyopsi yapılarak gerçekleştirilmiştir. Ortalama takip süresi medikal tedavi alanlarda 10-84 ay (ortalama 40 ay), cerrahi tedavi yapılan olgularda ise operasyondan sonra 9-71 ay (ortalama 32 ay) dır. En az 6 ay süreyle proton pompası inhibitörleri ve sisaprid ile tedavi olan 65 hastanın 10'unda (% 15.3) tanı konulduktan sonra 6-60 ay (ortalama 25 ay) içinde Barrett epiteli gelişmiştir. Anti-reflü cerrahi uygulanan hiçbir hastada ameliyat sonrası 9-71 (ortalama 32) ay takipleri neticesi Barrett epiteli gelişmemiştir. Asit azaltıcı ve prokinetik ajanlarla en az 6 ay süre ile yapılan tedavinin evre II ve III reflü özofajitli hastalarda barrett epiteli gelişimini engellemediğini gözlemlendi. Barrett epiteli gelişimini engellemek için reflüye karşı bariyer oluşturarak reflü nün engellenmesi, uygun bir yaklaşım olarak görünmektedir.

Abstract

Barrett epithelium replacement of esophagus is almost invariably associated with a severe degree long-standing gastro-esophageal reflux. Whether, the acid, bile or duodenal content of the refluxed material is important in the pathophysiology of this condition or its complication has yet to be determined. In this study we assesed the impact of acid reduction by a proton pomp inhibitor (omeprazole or lansoprazole) plus a pr akinetic agent (sisaprid) on prevention of columnar epithelial replacement in grade II and grade III reflux esophagitis and evaluated the results with anti-reflux surgery. Eighty-one patient with Savary grade II and III reflux esophagitis that were diagnosed between 1993-1999 were included in the study. Endoscopy and biopsy were the diagnostic procedure in all patients, and 24-hour ambulatory pH-meter was performed to 52 of the patients. Omeprazole 40 mg plus cisapride 40 mg were given to the first 40 patients, Lansoprazole 60 mg plus cisapride 40 mg were given to other 41 patients between 6-22 months (mean 13 months). Sixteen patients were operated for anti-reflux procedure after various time following medical treatment. Patients' follow up examinations were made by endoscopy and at least four quadrant biopsy. Mean follow up time for non operative group was 40 months (10-84 months). Specialized columnar epithelium and dysplasia were asked from the .pathologist. Ten patients in 65 (% 15.3) who were treated with a proton pump inhibitor plus cisapride at least 6 months developed Barrett epithelium between 6-60 months (mean 25 months) after diagnosis. No patients who were performed anti reflux surgery developed Barrett epithelium between 9-71 month (mean 32 months) follow up. Acid-reducing plus prokinetic agent treatment for at least 6 months seems not to prevent development of Barren's epithelium in grade II and III reflux esophagitis patients. In order to prevent this condition reflux must be prevented with a creation of a reflux barrier.
