Özet
Gebelikte laparaskopik cerrahi girişimler gün geçtikçe yaygınlaşmaktadır. Karbon dioksid (CO2 ) pnömoperitonunun anne ve fetüs fizyolojisinde yaptığı değişiklikler tam olarak bilinmediğinden anne ve fetus için ne kadar güvenilir olduğu hala tartışmalıdır. Gebelik peryodunun 110-140 günler arasında tekil gebeliği olan 8 koyuna anestezi verilereki ve entübe edildi. Bir adet trokar girişi yapılarak CO2 (n=4) veya helyum pnömoperitonunun (n=3) sabit basınçla. (14 mmHg) 45 dk süre uygulandı. lnsüflasyon öncesi (45 dk), insüflasyon süresince (45 dk) ve desüflasyon sonrası (45 dk) anne ve fetusdan veriler alındı. CO2 pnömoperitonunda kontrol verilerine göre anne ve fetusda anlamlı derecede PCO2 artışı (p>0.001) belir-lendi. CO2 pnömoperitonunda ileri derecede fetal asidoz gelişmesine karşın (p>0.001), helyum pnömoperitonunda asit-baz dengesinde anlamlı bir değişiklik olmadı. CO2 pnömoperitonunda değişen pH ve PCO2 değerleri desüflasyon periyodu sonunda, pnömoperiton öncesi değerlere geri döndü. Gebelikte laparoskopik girişimlerin başarılı olduğuna dair klinik çalışmalar bildirilmiş olmasına karşın, CO2 pnömoperitonu fetusda ileri derecede respiratuvar asidoza neden olmaktadır. Gebe hastalarda laparaskopik girişim gerekli olduğunda CO2 yerine helyum tercih edilmelidir.
Abstract
Laparoscopic surgical procedures are being performed in pregnant women with increasing frequency. Maternal-fetal physiologic changes occuring during intraabdominal carbondioxide CO2 insufflation are poorly understood, and maternal-fetal safety is of concern during CO2 pneumoperitoneum Eight ewes with single pregnancies between 110-140 days of gestation were anesthetized and intubated. A trocar was placed through the abdominal wall inflated with CO2 (n-4) or helium (n=3) at 14 mmHg pressure f or 45 min. Data were collected before insufflation (45 min), during insufflation (45 min) and after desufflation (45 min). During CO2 insuflation there was a significant increase (p>0.01) in maternal and fetal pCO2 comparison with prepneumo peritoneum. Fetal res-piratory acidosis occured, reproducibily, after establishment of CO2 pneumo peritoneum but did not occur before insufflation or under helium pneumo peritoneum (p>0.001). Despite clinical reports of succesful laparoscopic surgical procedure during pregnancy, significant respiratory aci-dosis may be induced in the fetus with CO2 pneumo. Helium is preferable to CO2 when performing laparoscopy in pregnant patients.
