Özet

Patolojik evrelemeden başka, güvenilir prognostik faktörler olmadığından dolayı, kolorektal kanserli hastalara postoperatif adjuvan tedavi uygulanmasına karar vermek güçtür. Çalışmanın amacı, kolorektal kanserlerde hastalıksız ve genel sağkalıma anjiogenezin etkisini evalüe etmek ve anjiogenez skoru ile hastalığın evresi, hastaların yaşı, tümör çapı ve grade'i arasında belirgin bir korelasyon bulunup bulunmadığını değerlendirmektir. Kolorektal kanserleri nedeniyle 1993 ve 1996 yılları arasında cerrahi müdahale uygulanan 93 hasta retrospektif olarak incelendi. Yaş, tümör çapı, hastalığın evresi, tümör grade'i, hastalıksız sağkalım ve genel sağkalım tümörün anjiogenez skoru ile karşılaştırıldı. Anjiyogenez skoru ile tümör çapı, hastalığın evresi, hastalıksız ve genel sağkalım arasında istatistiksel olarak anlamlı bir ilişki bulunurken, yaş ve tümör derecesi ile anlamlı bir ilişki yoktu. Kolorektal kanserlerde anjiyogenez skorunun patolojik evrelemeye ilaveten değerli bir prognostik faktör olduğu, tümör çapı ve hastalığın evresi ile yakından ilişkili olduğu sonucuna varıldı. Tümördeki anjiyogenetik aktivite adjuvan tedavi protokolü uygulama kararı için ilave bilgi verebilir.

Abstract

It is difficult to decide for applying postoperative adjuvant treatment to patients with colorectal cancer due to lack of any reliable prognostic factors other than pathological staging. The aim of the study is to evaluate the impact of angiogenesis on disease free and overall survival in colorectal cancers and to assess any significant correlation existence when angiogenesis score is compared to the stage of disease, patients age, tumour size and grade. Ninety-three patients who had surgical intervention for their colorectal carcinomas between 1993 and 1996 were reviewed retrospectively. Age, tumour size, stage of disease, tumour grade, disease free survival and overall survival were compared to angiogenesis score of the tumour. While a statistically significant correlation was found when angiogenesis score was compared to tumour size, stage of disease, disease free and overall survival, there was no significant relation when it is compared to age and tumour grade. It is concluded that angiogenesis score in colorectal carcinoma is a valuable prognostic factor in addition to pathological staging and it is closely related with tumour size and stage of the disease. Angiogenetic activity within the tumour can give additional information for decision making about adjuvant treatment administration.
