Özet

Bu retrospektif çalışmada 3 yıllık bir dönem boyunca Seydişehir Devlet Hastanesinde kriptoglandüler zeminde gelişen basit analfistül nedeni ile ameliyat edilen 20 olgu, rekürrens ve inkontinans oranlan ile birlikte değerlendirildi. Yüksek transsfinkterik, suprasfinkterik, ekstrasfinkterik, vb. kompleks fistüller çalışma dışı bırakıldı. Olguların 5 (% 25)'i kadın, .15 (% 75)'i erkek olup, ortalama yaş 43.5 olarak saptandı. Olgular ortalama 15 ay süre ile % 80 oranında takip edildiler. intersfinkterik ve/veya transsfinkterik iç de/iğin görülemediği ve düzensiz kontroller sonucu erken yara dış deliğinin kapanmasına ve yetersiz cerrahi tekniğe bağlı 3 (% 15) olguda rekürrens saptandı. Fekal kirlenme ve gaz inkontinansı şeklinde 3 (% 15) olguda geçici inkontinans gelişti. Basit olarak değerlendirilen bu tür füstüillerin bazen primer deliklerinin kolayca görülemediği ve sekunder traktlannm olabileceği ve bu nedenle aslında kompleks (submüsküler) olarak değerlendirilmeleri gerektiği sonucuna varıldı.

Abstract

A retrospective study of 20 patients with simple fistulas in-ano of cryptoglandular origin operated on over a 3-year period at Seydişehir State Hospital was undertaken for the purpose of establishing the recurrence and incontinence rates. More complex fistulas including high transsphincteric, suprasphinteric, extrasphincteric, ext. were excluded. There were 5 females and 15 (75 %) males, and average age was 43.5 years. Mean follow up was 15 months and followup rate was 80 %. Three (J5 %) patients developed recurrent fistulas, all of them had missed internal openings at the time of surgery, and one of them had premature fistulotomy wound closure due to irregular controls and inadequate surgical procedure. Temporary incontinence including fecal soiling and incontinence to flatus were seen at 3 (15 %) patients. All 'so called' simple fistulas in ami may not have readily detectable primary openings and may possess secondary tracts which preclude their behaviour as simple fistulas, thus may be called submusculary rather than simple fistula in ano.
