Özet
Erciyes Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalında 19852000 yıllan arasında safra kesesi karsinomu saptanan 52 hasta retrospektif olarak incelendi. 52 hastanın 32'si (% 61.5) kadın, 20'si (% 38.5) erkek, yaş ortalaması 59.4 (39-86) idi. En sık görülen semptomlar; sağ hipokondrium ağrısı (% 91.4), hulantıkusma (% 37.5), sanlık (% 25.4) ve kilo kaybı (% 21.2) idi. Tüm preoperatif muayene ve laboratuar tetkiklerine rağmen sadece 15 hasta (% 28.8) safra kesesi karsinomu ön tanısıyla operasyona alınmış; tanı 28 hastada (% 53.8) intraoperatif olarak, 9 hastada (% 17.4) ise postoperatif patolojik inceleme ile konulmuştur. Hastaların 17' sine (% 32.7) kolesistektomi, 6'sına (% 11.5) kolesistektomi+Ttüp yerleştirilmesi, 7'sine (% 13.4) kolesistektomi+karaciğer wedge rezeksiyon, 22'sine (% 42.3) ise inoperabilite nedeniyle sadece eksplorasyon+biyopsi yapıldı. Hastaların 33'ünde (% 63.4) safra kesesinde taş mevcuttu. TNM sınıflamasına göre 39 hasta (% 75) evre IV, 8 hasta (% 15.4) evre 111, 3 hasta (% 5.8) evre II ve 2 hasta da (% 3.8) evre l olarak saptandı. 9 hasta (% 17.4) postoperatif erken dönemde kaybedildi. Ortalama hastanede kalış süresi 16+3.4 gün, ortalama yaşam süresi 8.5 ay olarak belirlendi. Serimizde de görüldüğü gibi safra kesesi karsinomları hala prögnozıı en kötü GİS maligniteleri arasındadır. Bunun sebeplen; tümörün biyolojik davranışına, hastalık belirtilerinin geç ortaya yıkmasına ve benign safra kesesi hastalıklarına benzemesi nedeniyle tanının geç konmasına bağlıdır.
Abstract
52 patients who were diagnosed as gallbladder carcinoma in the General Surgery Department of Erciyes University Medical School between 1985 and 2000 were retrospectively analysed. 32 patients were male (61.5 %) and 20 were female (38.5 %). The mean age was 59.4 years (39-86 years). The most common symptoms and findings were as follows; right hypochondrium pain (91.4 %), nausea and vomiting (37.5 %), jaundice (25.4 %) and weight loos (21.2 %). Despite detailed preoperative physical examinations and all indicated laboratory procedures, only 15 patients (28.8 %) were preoperatively diagnosed as gallbladder carcinoma. The diagnosis was made in 28 patients (53.8 %) intraoperatively and in 9 patients (17.4 %) by postoperative pathological examination. The treatment approaches to the cases were as follows; cholecystectomy in 17 patients (32.7 %), cholecystectomy plus Ttube insertion in 6 patients (11.5 %), cholecystectomy plus hepatic wedge resection in 7 patients (13.7 %) and exploration plus biopsy in 22 patients who had inoperability criteria (42.3 %). 33 patients (63.4 %) had stones in the gallbladder. According to TNM classification, the patients were classified as follows; 39 patients (75 %) in stage IV, 8 patients (15.4 %) in stage III, 3 patients (5.8 %) in stage II, 2 patients (3.8 %) in stage I. 9 patients (17.4 %) died in the postoperative early period. The mean hospital stay duration was 16±3.4 days and mean survival duration was 8.5 months. As seen in our case series too, gallbladder carcinomas are still one of gastrointestinal system malignities with the worst prognosis. The reasons of such a bad prognosis are late diagnosis because of the clinical similarity to the benign gallbladder diseases, biological behavior of the tumour and late presentations of the symptoms of the disease.
