Postoperatif analjezi için tek doz intratekal morfin kullanımı

Özet
Bu çalışmada, hemoroidektomi operasyonlarında, intratekal morfin kullanımının postoperatif analjezi kalitesi ve komplikasyonlara olan etkisini araştırmayı amaçladık. Çalışmamızda ASA 1-11 grubunda yaşlan 18-66 arasında 43 hasta rasgele 3 gruba ayrıldı. Grup I (n= 15)'deki hastalara 7.5 mg hiperbarik bupivakain+0.1 mg morfin, grup II (n= 15)'deki hastalara 7.5 mg hiperbarik bupivakain+0.2 mg morfin, grup III (Kontrol grubu) (n= 13)'de-ki hastalara 7.5 mg hiperbarik bupivakain+0.2 mL serum fizyolojik verildi. Gruplar arasında yaş, kilo, boy ve cinsiyet açısından anlamlı fark yoktu. Üç grupta da cerrahi için sensoryal blok seviyeleri ve analjezi kaliteleri yeterli idi. Sensoryal blok seviyeleri ve bu seviyelere ulaşma zamanları açısından, gruplar arasında anlamlı farklılık yoktu (p>0.05). Gruplar arasında blok öncesi, intraoperatif, postoperatif kan basıncı, kalp atım hızı değişiklikleri, periferik oksijen satürasyonu değerleri, solunum sayılan yönünden istatistiksel olarak anlamlı fark yoktu (p>0.05). Hiçbir grupta solunum sayısında anlamlı olarak düşme ve solunum depresyonu görülmedi. Postoperatif İADZ; grup l'de 11 ±4.52, grup II'de I2.33±4.06, grup III'de 2.46±0.52 saat idi. Grup l ve grup Il'nin İADZ, grup 111'e göre istatistiksel olarak anlamlı derecede fazla idi (p0.05). Postoperatif idrar retansiyonu (PIR) yönünden gruplar arasında anlamlı fark yoktu. Abstract
In this study, we planned to evaluate the rate of complications and the quality of postoperative analgesia of morphine applied by intratechal in hemorrhoidectomy procedures. ASA(according to the American Society of Anest-hesiologists) class 1-H patients (n=43) aged 18-66 years divided randomly 3 groups. In group I (n=IS); 7.5 mg hyperbaric bupivacaine plus O.I mg morphine, group 11 (n=l5); 7.5 mg hyperbaric bupivacaine plus 0.2 mg morphine and in group III (control group) (n=13); 7.5 mg hyperbaric bupivacaine plus 0.20 mL saline were applied. There were no significant difference about age, weight, height and gender. The levels of sensorial block and analgesia were adequate for surgery in each three groups. There were no significant difference between the groups about the levels of sensorial block and time (p>0.05). There were no significant difference between the groups about the blood pressures, pulse rate, SpC>2 and respiration rate measured before the block, during and after the operation (p>0.05). None of the groups showed respiratory depression or significantly decrease in respiration rate. Postoperative FPTP was 11 ±4.52 hours in group 1, J2.33±4.06 hours in group II and 2.46+0.52 hours in group 111. FPTP's of groups I and U were significantly higher than the one of group III (p0.05). There was no significant difference between the groups for frequency of urinary retention (p>0.05). The rate pruritus of group II and I was higher than the one of group III (respectively, % 33, % 27, % 0, p0.05). The rates of vomiting and nausea were significantly high-er in group I and II than group III (p0.05). In this study, we determined that 0.1 mg of intratechal morphine was adequate to supply early postoperative analgesia with minimal complications in hemorrhoidectomy operations.
