Özet

Askaris İnfestasyonunun cerrahi girişim gerektiren komplikasyonlarını sergilemek, klinik özellikleri, tanı ve tedavi yaklaşımlarını tartışmak amacıyla, 1975-2000 yılları arasında kliniğimizde opere edilen, 26 hastanın verilerini retrospektif olarak irdeledik. On altı erkek, on kadın hastanın on dördü (53.8 %) erişkin, onikisi (46./ %) çocuk idi. Klinik tablo 23 hastada barsak tıkanıklığı, 2'sinde granulomatöz peritonit, l'inde kolanjit şeklinde idi. Barsak tıkanıklığı sıklıkla (n:18) basit yapıda idi. Komplike tıkanıklığı olanların 4'ünde volvulus, 1'inde intussııssepsiyon mevcut idi. Cerrahi işlem olarak tıkayıcı parazit yumağının manuel olarak ilerletilmesi 7, enterotomi 12, enterotomi ve gastrotomi l, intestinal rezeksiyon 3 hastada uygulandı. Kolesistektomi, koledokotomi ve safra yatları eksplorasyonu kolanjiti olan, drenaj ise peritoniti olan olgularda uygulanan cerrahi işlemler idi. Preoperatif tanı sadece olguların % 27'sinde mümkün oldu. Morbidite ve mortalite oranlan % 30.8 ve % 11.5 şeklinde idi. Gastrointestinal veya pankreatikobiliyer kanalın tümör, taş ya da dıştan bası gibi faktörlerin ortaya konamadığı hemen tüm mekanik tıkanıklıklarında, özellikle de hasta kötü hijyen ve düşük sosyoekonomik standartların olduğu bölgelerden ise, askaris infestasyonu hatırlanmalıdır.

Abstract

The records of 26 patients, admitted with the diagnosis of complicated ascaris infestations and treated by surgery, were reviewed in order to define their clinical aspects and discuss the diagnostic and therapeutic approaches. There were 16 men and 10 women. Fourteen (53.8 %) were adults and 12 (46.1 %) were children. Presentation was as intestinal obstruction in 23, ascaris peritonitis in two and cholangitis in one of them. Intestinal obstruction was mostly in simple form (n:l8). Of 5 patients with complicated obstruction 4 had volvulus, 1 had intussusception. Surgical interventions were extraluminal manual advancement of the obstructing parasitic bundle in 7, enterotomy in 12, both enterotomy and gastrotomy in I, intestinal resection in 3 patients with gangrenous bowel. Cholecystectomy, choledochotomy and bile duct exploration was the procedure performed in the patient who had cholangitis, drainage in the patients with granulomatous peritonitis. Preoperative diagnosis could only be made in 27 % of the patients. Morbidity and mortality rates were 30.8 % and 11.5 %, respectively. Since it is difficult to identify ascariasis before surgery, clinician elsewhere should think the probability of ascariasis whenever she or he diagnose a mechanical obstruction in gastrointestinal or biliopancreatic tract in a patient from a region of low socioeconomic conditions and poor hygiene.
