Piloroplastili ve piloroplastisiz özofajektominin karlşılaştırılması

Özet
Özofagus rezeksiyonu sonrası intestinal devamlılığın mide ile sağlandığı olgularda vagal denervasyondan dolayı piloroplasti veya piloromiyotominin gerekliliği sorgulanmaktadır. Bu drenaj yöntemlerinden sonra yaşam kalitesini etkileyen semptomatik reflü ve regurjitasyon gibi problemler ortaya çıkabilmektedir. 1994-1999 yıllan arasında J3'ü kadın, 20'si erkek olmak üzere 33 hastaya transhiatal özofajektomi ve gastrik pullup ameliyatı yapılmıştır. İleri özofajite bağlı gelişen striktiirlii bir hasta hariç diğer bütün hastalar özofageal kanser nedeni ile ameliyat edilmişlerdir. Otuzüç hastadan 9 hastaya piloroplasti yapılırken 2 hastaya piloromiyotomi uygulanmıştır. Erken postoperatif dönemde nasogastrik aspirasyon miktarını ve oral'beslenmeye karşı tolerans değerlendirmeye alınmıştır. Bütün hastalar 6-20 ay boyunca takip edilerek sorgulanmıştır. Postoperatif dönemde günlük ortalama nasogastrik aspirasyon miktarı ve oral beslemeye başlama zamanı drenajsız ve drenajlı grup arasında farklılık göstermedi (50 mL'ye karşı 40 mL ve 4.3 gün e karşı 5.1 gün). Hastanede kalış süreleri bakımından da (12.6 gün/13.5 gün) belirgin farklılık tespit edilmedi. Hastanede kaldıkları sürece her iki gruptaki hastalar ne reflüden ne de regürjitasyondan yakındılar. Piloroplastili 5 hastada ("h 62) postoperatif 8. haftada semptomatik bilier reflü ve/veya regurjitasyon saptandı. Drenaj yapılmayan hastalarda reflü, regurjitasyon veya mide çıkış obstrüksiyonuna bağlı şikayetler yoktu. Ameliyattan sonraki 12. ayda, drenaj uygulanmış 5 hastanın 4'ünde reflü ve/veya regurjitasyon yakınmaları bulunurken, drenaj yapılmayan 6 hastanın hiçbirisinde bu yakınmalara rastlanmamıştır. Bu grupta 2 hastanın arasıra distansiyon ve kusma şikayetleri olmuştur. Bu hastalar balon ile pilor dilatasyonuna iyi cevap vermişlerdir. Özofageal striktürlü (piloroplasti veya miyotomisiz) hastanın 20 aylık takiplerinde reflü, regurjitasyon ve mide çıkış obstrüksiyonu görülmemiştir. Özofagus rezeksiyonunu takiben mide ile intestinal devamlılığın sağlandığı vakalarda piloroplasti yapmak zorunlu değilmiş gibi görünmektedir. Drenajın yapılmaması hu hastalarda hayat kalitesini olumsuz yönde etkileyen bilier reflü ve regurjitasyon gibi problemleri engelleyebilir.
Abstract
After resection of esophagus and gastric pullup procedure, pyloropasty or pyloromyotomy is felt to be necessary due to vagal denervation. But symptomatic reflux and régurgitation are problems affecting quality of life following these procedures. In this study we compared patiens who were performed pyloroplasty with those who were not performed pyloroplasty following transhiatal esophagectomy and gastric pull up procedure. Between 1994-1999, transhiatal esophagectomy and gastric pullup uas performed in 33 patients (13 female, 20 male). Esophageal carcinoma was the reason in all patients except one who was performed esophageal resection for stricture following esophagitis. Pyloroplasty were done in 9 patients and pyloromyotomy in 2 patients as a drainage procedure. No drainage and dilatation procedure during operation was performed to the other 22 patients. Patients were followed up between 6-20 months. One patient in each group died from unrelated reasons in early postoperative period. Postoperative mean daily nasogastric aspiration (50 vs 40 mL) and the time starting oral feeding (after 4.3 days for nondrainage group vs 51 days for drainage group) were not significantly different between groups. Difference in hospital stay was also insignificant (12.6 days vs 13.5 days). No patient in both groups complained reflux or regurgitation during hospital stay. After 8 weeks postoperatively all patients attended follow up controls and 5 patients with pyloroplasty (62 %) had symptomatic biliary reflux and/or regurgitation. No patients in non drainage procedure complained of reflux, regurgitation or gastric outlet obstruction during same period. After 12 months, in pyloroplasty group 4 patients out of 5 complained of reflux and/or regurgitation no reflux or regurgitation noted in 6 non drainage patients seen during same period. Two patients in this group complained occasional distention and vomiting. They responed well to balloon dilation of pylorus. The patient with esophageal stricture (without pyloroplasty or myotomy) was followed up 20 months with no reflux regurgitation and gastric outlet obstruction. Pyloroplasty following esophageal. resection and gastric pullup seems not to be essential.Without drainage procedure biliary reflux and regurgitation that affects these patients' quality of life may be prevented?
