Prolen mesh ile yapılan gerilimsiz fıtık onarımında (Lichtenstein), Visuel Analog Skalayı (VAS) kullanarak, preemptiv bupivakain HCL ve preoperatif tenoksikamın, yalnız ve kombine postoperatif ağrı üzerine etkileri Nyhus sınıflandırmasına göre Tip3A ve B hernisi olan 3 grup hastada karşılaştırılmıştır. Randomize olarak, I. gruba preoperatif plasebo, 2. gruba preoperatif tenoksikam, 3. guruba ise preoperatif tenoksikam ve preinsizyonel bupivakain infiftrasyonu yapılmış ve postoperatif 1,6,12,24. saatlerde VAS ile çift kör değerlendirilmişlerdir. Ek analjezi açısından istatiksel olarak değerlendirildiklerinde; preemptiv süpozituar tenoksikamın, plesebo gurubuna göre 6. saatten sonra anlamlı VAS düşüklüğü, tenoksikam+bupivakain uygulamasının ise l. saat'ten haşlayarak VAS değerinde anlamlı iyileşme ve ek analjezi gereksiniminde anlamlı azalma sağladığı görülmüştür. Tenoksikam ile tenoksikam+bupivakain gurubu arasında I. saat'te VAS skoru ayısından anlamlı fark gözlenirken, 6. saat sonrası bu farkın kaybolduğu anlaşılmıştır. Prolen mesh ile gerilimsiz fıtık onarımında, preinsizyonel. bııpivakaine ve rektal tenoksikamm preemtiv amaçla kullanımının, postoperatif ağrıyı pleseho guruba göre anlamlı ölçüde azalttığı ve bu azalmanın tek başına tenoksikam kullanımına göre 1. saatte olumlu fark oluşturduğu saptanmıştır.

Abstract

The effects of preemptive bupivacaine HCI and preoperative tenoxicam either used as a single agent or in combination on postoperative pain perception in 3 groups of patients with Type 3A and B hernia according to Nyhus Classification were compared using Visual Analogue Scale following tension free hernia repair with prolen mesh (Lichtenstein). In a randomized fashion, 1st group received placebo, 2nd group received preoperative tenoxicam and 3rd group received preoperative tenoxicam and preincisional bupivacain infiltration. The groups were evaluated in 1st, 6th, 12th, and 24th hours in a double blind fashion using VAS. After statistical analysis of the groups regarding additional analgesia, it was found that preemptive suppository tenoxicam group had significantly lower VAS values when compared with placebo group. It was also found that application of tenoxicam+bupivacain resulted in a significant decrease in VAS values and additional analgesia need after the 1st hour of application. Although a significant difference was observed between VAS scores of tenoxicam and tenoxicam+bupivacain groups after one hour, such a difference could not be observed after six hours. It was found that administration of preincisional bupivacaine and rectal tenoxicam for preemptive purposes in tension free hernia repair with prolen mesh resulted in a significant amount of decrease in postoperative pain perception in comparison to placebo and. this decrease was also statistically more significant than the only tenoxicam group after the first hour of administration.
