Hemodiyaliz hastalarında değişik tip arterio-venöz fistül pansumanlarının değerlendirilmesi

Özet
Arter io Venöz Fis tüller in diyaliz uygulamalarında en güvenli ve uzun süreli kalıcı damar yolu olması nedeniyle korunması ve işlerliğinin devam etmesi son derece önemlidir. Lokal dejenerasyonlarla ilgili problemlerden biri de tekrarlayan iğne girişlerinin erozyonlara yol açması ve bu bölgenin lokal irritasyonlar ve enfeksiyonlara açık olmasıdır. Bu yüzden A VF giriş bölgelerinde değişik tip pansumanlar uygulandı. Hastalar 3 gruba ayrıldı. Birinci gruba (Grup I, n: 10) sadece "Battikon", ikinci gruba (Grup II, n: 10) "Battikon + Calsium Pantothenatepomad" ile pansuman, üçüncü gruba (Grup III, n: 10) "Battikon + Heparinoid-Hyaluronidase pomad" ile pansuman uygulandı. Gruplar 12 haftalık periyot içinde AVF'nin klinik durumu (deri erozyonu, lokal enfeksiyon, sızıntı ve tromboz yönünden) ve arter-ven kompresyon süreleri, pıhtılaşma zamanları ve trombosit değerleri yönünden değerlendirildi. Sonuçta pansuman materyalleri açısından sadece "Battikon" ile pansuman yapılan hastalarda daha çok deri erozyonu (% 5.8) ve sızıntı (% 8.3) olduğu görüldü. Diğer parametreler açısından gruplar arasında anlamlı bir farklılık yoktu. A V fıstüllerin pansumanında pozitif koruyucu etkilerinden dolayı bir pomad eklenmesi gerektiği kanısına vardık.
Abstract
Because of the Arterio-Venous Fistula is the safest and the longest lasting permanent vascular access in the hemodialysis patients, protecting and the survival of the fistula is so important. One of the problems of the local degeneration is the multiple vein punctures which leading skin erosions. Also this skin entry site is open to local irritations and infections. Therefore, different types of local dressing is performed on the needle skin entery site in 30 hemodialysis patients. The patients were divided into three groups. In the first group (Group I, n: 10) local dressing were done by "Batticon" only. In the other groups (Group II, n: 10) "Batticon + Calcium Pantothenate ointment" and (Group HI, n: 10) "Batticon + Heparinoid Hyoluronidase ointment" were used. In 12 weeks period we examine the complications of the A Vfistulas and arterio-venous compression times, clotting time and platelet values. In conclusion, in group I the local laceration (5.8 %) and leakage (8.3 %) complications were seen more often than the other groups. Also there is no significant differences between the groups on the other parametres. Thus we say that in the dressing of the AVfistulas we have to use the ointment treatment for the positive protective effects of them.
