Lipoprotein (a ) düzeyi ve diğer lipid parametreleri arasındaki ilişkiler

Özet

ÖZET. Lipoprotein(a) aterojenik bir lipoprotein olup, son yıllarda kardiyovasküler hastalıklar için önemi artan bir risk faktörü haline gelmiştir. Diğer yandan plazma Lp(a) konsantrasyonlarını etkileyen faktörler henüz tam olarak anlaşılamamıştır. Bu nedenle biz de plazma Lp(a) düzeyi ile diğer lipid parametreleri arasındaki ilişkiyi araştırmak amacıyla 110 olguda total kolesterol, HDL kolesterol, trigliserid, Apo A, Apo B ve Lp(a) düzeylerini inceledik. Olgular trigliserid ve kolesterol düzeylerine göre 4 gruba ayrıldılar. Kolesterol >200 mg/dl ve trigliserid >200 mg/dl olan olgular 1. grup (n= 22), kolesterol 200mg/dl olan olgular 2. grup (n= 22), kolesterol > 200 mg/dl ve trigliserid < 200 mg/dl olan olgular 3. grup (n= 35) ve kolesterol < 200 mg/dl ve trigliserid < 200 mg/dl olan olgular 4. grup (n= 31) olarak sınıflandırıldı. Plazma Lp(a) düzeylerinin ortalama ve ortanca değerleri 1. grup için 33 mg/dl ve 13 mg/dl, 2. grup için 22 mg/dl ve 17 mg/dl, 3. grup için 51 mg/dl ve 32 mg/dl, 4. grup için 25 mg/dl ve 19 mg/dl olarak saptandı. Trigliseridi normal olan 3. grup ile trigliseridi yüksek olan 1. ve 2. grup arasında Lp(a) düzeyi bakımından anlamlı farklılık vardı (sırasıyla p< 0.05 ve p< 0.05). Kolesterol ve trigliserid düzeyi normal olan olgular hariç tutulduğunda diğer tüm hiperlipidemik olgularda Spearman korelasyon testi ile Lp(a) ile trigliserid düzeyi arasında negatif bir korelasyon saptandı (r= -0.22, p < 0.05). Total kolesterol, HDL kolesterol, Apo A ve Apo B ile Lp(a) düzeyleri arasında ise bir ilişki saptanmadı. Sonuç olarak hiperlipidemili olgularda trigliserid ile Lp(a) arasında negatif bir korelasyon saptanırken, diğer lipid parametreleri ile Lp(a) arasında anlamlı farklılık saptanmadı. Muhtemelen hipertrigliseridemi trigliseridden zengin Up (a) klirensini arttırarak Lp(a) düzeyinin düşmesine neden olmaktadır. Bu durum, plazma Lp(a) düzeyinin regülasyonunda trigliseridin önemli rolü olabileceğini düşündürmektedir.

Abstract

SUMMARY. Lipoprotein (a) is an atherogenic lipoprotein and in recent years it is becoming a major risk factor for the cardiovascular disease. Factors modulating plasma Lp(a) concentrations are poorly understood yet. Therefore in this study to investigate the relationships of plasma Lp(a) levels with other lipid parameters, total cholesterol, HDL cholesterol, triglyceride, Lp(a), Apo A and Apo B levels were measured in 110 patients. The patients were divided into 4 groups according to cholesterol and triglyceride levels. Patients who were both hypercholesterolemic and hypertriglyceridemic were accepted as the first group (n= 22), normocholesterolemic and hypertriglyceridemic patients as the second group (n= 22), hypercholesterolemic and normotriglyceridemic patients as the third group (n= 35), normocholesterolemic and normotriglyceridemic patients as the fourth group (n= 31). The mean and median plasma Lp(a) levels were 33 mg/dl and 13 mg/dl in the first group. 22 mg/dl and 17 mg/dl in the second group, 51 mg/dl and 32 mg/dl in the third group and 25 mg/dl and 19 mg/dl in the fourth group. There were significant difference between the third group whose triglyceride levels were normal and the second and the third group whose triglyceride levels were high (p< 0.05 andp< 0.05 respectively). Excluding the normocholesterolemic and normotriglyceridemic cases, in all of the hyperlipidemic patients there were significant negative correlation betveen triglyceride andLp(a) levels with Spearman correlation test (r= -0.22, p< 0.05). There were no relation among Lp(a) and total cholesterol, HDL cholesterol, Apo A and Apo B levels.In conclusion Lp(a) levels were found to be negatively correlated with triglyceride levels in hyperlipidemic cases, while there wasn't any significant relation found among the other lipid parameters. It is possible that hypertriglyceridemia leads to decrease of Lp(a) levels by increasing the clerance of triglyceride rich Lp(a). This situation indicate that hypertriglyceridemia may have a role in the regulation of Lp(a) levels.
