Özet

Göz yaralanmaları adli tıp uzmanlarının sıklıkla karşılaştıkları, raporlarda hayati tehlike ve mutat iştigal sürelerinin yanı sıra genellikle uzuv zaafı ya da tatili ile çehrede sabit eser açısından değerlendirilmesi gereken lezyon-lar içermesi ve raporların travma öncesi mevcut bulguların da değerlendirilerek düzenlenmesi nedeni ile problem yaratan yaralanmalardır. Çalışmamız, göz yaralanması olgularının adli tıp açısından değerlendirilmesi ve değerlendirme aşamasındaki zorlukların tartışılması amacıyla planlandı. Adli raporlarının düzenlenmesi için Ocak 1995- Aralık 2001 tarihleri arastndaki 7 yıllık dönemde Uludağ Üniversitesi Tıp Fakültesi Adlî Tıp Anabilim Dalı'na gönderilen 3593 olguya ait evraklar retrospektif olarak incelendi. Göz yaralanması nedeni ile haklarında rapor istenen 135 olgu (%3.76) için düzenlenen 152 adli rapor gönderilen makam, yaralanma sebebi, lezyonlu göz, lezyonun yeri ve sonuçları yönünden değerlendirildi. Olguların IIVi erkek, 24'ü kadın olup; 127'sinin kat'i rapor, 8'inin maluliyet tayi¬ni için gönderildiği tespit edildi. En çok yaralanan gözün 61 olguda sol göz olduğu; 46 olguda sağ gözün, 28 olguda da her iki gözün yaralandığı; orijin olarak 57 olgu ile trafik kazalarının ilk sırayı aldığı; 66 olguda yaralanma sonucunda perforasyon geliştiği belirlendi. Rapor sonuçlarına göre 54 olgunun (%40) 11 -19 gün mutat iştigaline engel teşkil eden; 58 olgunun (%42.96) ise uzuv tatili meydana getiren nitelikte yaralanmaya maruz kaldığı tespit edildi. Gözler anatomik ve fizyolojik olarak iyi korunmalarına rağmen, oldukça sık karşılaşılan göz yaralanmalarının değerlendirilmesi aşamasındaki zorluklar nedeniyle; raporları, gelişen tıbbi teknikler kullanılarak göz hastalıkları uzmanları ile adli tıp uzmanlarının birlikte düzenlemeleri gerektiği sonucuna varıldı.

Abstract

Ocular traumas were encountered frequently by the forensic medicine specialists and ocular traumas have problems because of including the evaluation of injury which do or not endanger life, which renders the victim unable to follow his ordinary pursuits for a period, functionally or anatomically failing or missing the organ, face constant sign in their reports and arranging the reports with the evaluation of signs before the injury. In our study, we planned to evaluate the ocular trauma cases and discuss the complexity of their evaluation in the aspect of forensic medicine. The medicolegal reports of the 3593 cases that referred to the Forensic Medicine Department of Uludağ University Medical Faculty between January 1995 and December 2001 in the period of 7 years were examined retrospectively, and 135 cases (3.76%) of them were analysed by the sender office, type of injury, injured eye, localization of the injury and results, 152 medicolegal reports were written in these 135 cases (111 male and 24 female ). It was asked for definite report in 127 cases and disability rate in 8 cases. The most injured eye was the left with 61 cases, in 46 cases the right eye was injured; it was injured bilaterally in 28 cases. The most cause of the injuries was traffic accidents with 57 cases and in 66 cases perforating injuries were determined. In 54 cases (40%), it was determined the injury which rendered the victim unable to follow his ordinary pursuits for a period lasting from II to 19 days. The number of cases with the severity of missing on organ was 58 (42.96%). In conclusion, although the eyes were highly protected anatomically and physiologically, the ocular traumas were encountered frequently and we are considering the medicolegal reports of the ocular trauma cases should be arranged by the togetherness of the forensic medicine specialists and the ophthalmologs by using advanced medical techniques, because of the complexity of their evaluation in the aspect of forensic medicine.
