Özet

Hipertrofik kardiyomiyopati (HKMP)'li hastalarda beta-blokerlerin efor kapasitesi ve kardiyak otonomik fonksiyonun göstergesi olan kalp hızı değişkenliğine (KHD) etkilerini araştırmak amacıyla 14 non-obstrüktif HKMP hastası çalışmaya alındı. Çalışmanın ilk aşamasında (faz I) hastaların kullanmış oldukları beta-bloker ve benzeri etkileri olan kalsiyum kanal bloker tedavileri kesildi. Tedavinin kesilmesini takip eden dördüncü günde hastalara Hoher takılarak semptom ile sınırlı modifiye-Bruce protokolünde efor stres testi uygulandı. Test sonrasında tüm hastalara tol-ere ettikleri en yüksek dozda metoprolol başlandı. Hastalar ortalama bir ay sonra tekrar çağrılarak çalışmanın ikinci aşamasına (faz II) geçildi. Aynı protokol beta-bloker tedavisi altında uygulandı. Beta-bloker tedavisi ile hastaların efor kapasiteleri 7.7±2.6 MET'den 9.5±2.3 MET'e (p=0.03); efor süreleri ise 676+190 saniyeden 870±129 saniyeye yükseldi (p=0.005). Egzersiz sırasında normalize edilmiş düşük frekans değerleri [normalized unit of low frequency; LF(nu)], beta-bloker tedavisi ile 29.8±2.1'den27.5±l.2'ye geriledi (p=0.03). KHD'nin diğer komponentlerinde anlamlı değişiklik saptanmadı. Sonuç olarak beta-bloker tedavisi, HKMP hastalarının efor süre ve kapasitelerini artırırken, sempatik modülasyonun komponenti olan LF(nu) değerlerini azalttı.

Abstract

Fourteen patients with hypertrophic cardiomyopathy (HCMP) were enrolled into the study in order to examine the effect of beta-blockers on exercise capacity and cardiac autonomic function in patients with non-obstructive HCMP. At the initiation phase (phase I) beta-blocker and calcium antagonists were stopped in all patients. At 4th day after the withdrawal of the drugs exercise stress test in modified Bruce protocol was performed with simultaneous rhythm Hoher monitorization to all patients. After the test metoprolol was started in maximal tolerated doses. After 1 month the patients were invited to the visits and the second stage of the study begun (phase II). The same protocol was applied during beta-blocker therapy. With the beta-blocker therapy the exercise capacity and the exercise duration of the patients increased from 7.7 ± 2.6 METs to 9.5 ±2.3 METs (p=0.03) and from 676 ± 190 s to 870 ± 129 s (p=0.005), respectively. During exercise, with beta-blocker therapy, the normalized unit of low frequency fLF(nu)] decreased from 29.8 ± 2.1 to 27.5 ±1.2 (p=0.03). The other components of heart rate variability did not change significantly. In conclusion with beta-blocker therapy the total exercise duration and capacity increased and the component of sympathetic modulation; the LF (nu) decreased in patients with hypertrophic cardiomyopathy.
