Özet
Amaç: Bitlis Devlet Hastanesinde son yıl bir yıl içinde acil şartlar altnda ameliyat edilen kraniyalolguların retrospektf irdelenmesi Gereç ve Yöntemler: Bu çalışmada Mayıs 2011 - Mayıs 2012 tarihinde sadece acil şartlar altndakraniyal nedenlerle ameliyat edilen olgular retrospektf olarak değerlendirildi. Kafa travmalarınaek olarak spontan intraserebral hematom, subdural hematom, intraserebellar hematom gibi acilnedenlerle ameliyat edilen olgular çalışmaya dahil edildi. Elektf kraniyal girişimler çalışma dışındatutuldu. Hasta yaşı, cinsiyet, başvuru Glasgow koma skalası (GKS), ameliyat öncesi tanılar ve sondurum not edildi.Bulgular: Çalışmaya 78 erkek 30 kadın hasta dahil oldu. 17 hasta travma dışı nedenle ameliyat edil -di. Erkeklerde en sık neden trafk kazası iken kadınlarda en sık neden düşme olarak tespit edildi. Ensık acil ameliyat nedeni travmatk epidural hematom idi. Hastaların başvuru esnasındaki GKS de -ğerlerine bakıldığında hastaların %47,2sinde GKS değeri 8 veya altndaydı. Mortalite oranı %21,3olarak tespit edildi. Mortalite ile başvuru esnasındaki GKS arasında anlamlı ilişki varlığı saptandı. Sonuç: Nöroşirurji kliniklerinde travma dışı nedenlerle yapılan acil operasyonlar azımsanamayacakbir oranı oluşturmakta ve çalışmamızda olduğu gibi yüksek mortaliteye katkıda bulunmaktadır.
Abstract
Objectve: To evaluate patents who was been operated under emergent conditons for cranialcauses in Bitlis state hospital during last year. Methods: In this study, patents who was been operated only under emergent conditons forcranial causes between May 2011 and May 2012 were evaluated retrospectvely. In additon tohead trauma, patents with emergent cranial causes like spontaneous intracerebral hematoma,subdural hematoma, intracerebellar hematoma were also included in this study. Electve cranialoperatons were excluded. Age of the patents, gender, inital Glasgow coma scale, preoperatvediagnoses, and last status of the patents were noted. Results: 78 male and 30 women were included in the study. 17 patents were operated fornon traumatc causes. It was found that the most common reason for head trauma was trafcaccident in men and fallls in women. The most common cause of operaton was traumatc epiduralhematoma. Inital Glasgow coma scale was equal and lesser than 8 in 47,2 % of patents. Totalmortality rate was 21,3%. There was a signifcant relatonship between inital Glasgow coma scaleand mortality.Conclusion: The rate of emergent operatons for non traumatc causes in neurosurgery should notbe underestmated because it may contribute to high mortality rates as in our study.
