Magnetic Resonance Imaging Findings of Pediatric Epilepsy Patients in Afyon Region

Özet

Amaç: Epilepsi, kortikal gri cevherden kaynaklanan anormal elektriksel deşarj nedeniyle gelişen tekrarlayıcı epileptik nöbetlerle karakterizedir. Çalışmamızda Afyon bölgesinde epilepsi nedeniyle araştırılan pediatrik hastaların beyin manyetik rezonans görüntüleme (MRG) bulgularını ve epilepsi ile ilişkili sık görülen patolojileri ortaya koymayı amaçladık.Gereç ve Yöntem: Afyon Kocatepe Üniversitesi Tıp Fakültesi Hastanesi'nde 2012 ile 2015 yılları arasında epileptik nöbet araştırılmak üzere radyoloji ünitesine yönlendirilen ve beyin MRG yapılmış olan 0-18 yaş grubunda toplam 206 hasta çalışmaya dahil edildi. Hastaların MRG raporları hastane arşivinden retrospektif olarak tarandı. Epilepsi etyolojisinde rol oynayabilecek tüm bulgular kaydedildi. Tüm hastaların klinik bulguları ve elektroensefalogramları (EEG) epilepsi tipini belirlemek üzere pediatrik nörolog tarafından değerlendirildi.Bulgular: Çalışmaya dahil edilen olguların 98'inde (%45.4) MRG'de epilepsi etyolojisinde rolü olabilecek bir veya birkaç lezyon bir arada bulundu. Hastaların 108'inde (%55.6) MRG bulguları normal olarak değerlendirildi. Hastalarımızın yaş ortancası 7 olup toplam 106 kız (%51.5) ve 100 (%48.5) erkek hasta bulunuyordu. Tesbit edilen bulgular arasında ilk üç sırayı, edinilmiş hasar, kortikal gelişim anomalileri ve malformasyonlar ile yer kaplayıcı lezyonlar paylaşmaktaydı.Sonuç: Son yıllarda geliştirilen yeni görüntüleme tekniklerinin birleştirilerek kullanıldığı füzyon görüntüleme metodları ile rutin MRG'de saptanamayan epileptojenik odaklar görülebilir hale getirilmektedir. Görüntüleme ünitemize bu tekniklerin eklenmesiyle epileptik hastalarda lezyonların karakterize edilmesinde MRG'nin katkısının artacağını düşünüyoruz.

Abstract

Objective:Epilepsy is characterized by recurrent seizures due to abnormal electrical discharge of cortical graymatter. In our study, we aimed at exposing common brain magnetic resonance imaging (MRI) findings and pathologie sassociated with epilepsy in pediatric patients in Afyon region.Materials and Methods: A total of 206 patients were enrolled in the study between 0-18 ages in Afyon Kocatepe University Medical Faculty Hospital, between 2015 and 2012 that were referred to the radiology unit to investigate seizures with MRI. MRI reports of the patients were obtained from hospital archives retrospectively. All findings that could play a role in the etiology of epilepsy were recorded. All patients' clinical and electroencephalogram findings (EEG) were evaluated by a pediatric neurologist to determine the type of epilepsy.Results: In 98 of the cases included in the study (45.4%), a combination of one or more lesions which may play a role in the etiology of epilepsy were found on MRI. In108 patients (55.6%) no abnormal MRI findings were explored. The median age of the patients was 7 , 106 (51.5%) of them were female and 100 (48.5%) were male. The first three abnormal findings on MRI were; acquired damage, anomalies and malformations of cortical development and, space-occupying lesions.Conclusion: In recent years, MRI fusion imaging methods using new imaging techniques are applied to determine epileptic foci more sensitively than routine imaging methods. We believe that with the addition of this technology to our imaging unit, thec ontribution of MRI in characterizing lesions in epileptic patients will increase.
