Mycobacterium Bovis ile enfekte bir inekte saptanan granulomatöz lezyonlar

Özet
Afyonkarahisar yöresinde, 9 yaşlı Montofon ırkı, daha önceden tedavi görmüş bir inek kronik öksürük, mukoprulent burun akıntısı, aşırı zayıflama şikayetiyle kliniğe getirildi. Klinik muayenede hayvanda vücut sıcaklığı 39,1ºC, solunum sayısı 43/dk ve kalp frekansı 77/dk tespit edildi. Öksürük, anoreksi, aşırı zayıflama, burunda mukoprulent bir akıntı ve solunum güçlüğü görüldü. Klinik muayeneler sonucu inekte tüberküloz olabileceği düşünülerek “Sığır Tüberkülozu Yönetmeliğine” göre alerjik bir test olan tüberkülin uygulaması sonucu pozitif çıkan hayvan kesime gönderildi. Kestirilen hastada makroskopik muayenede göğüs kafesinin iç yüzeyinde parietal plörada 2-5mm arasında değişen büyüklükte sert kıvamlı, boz-beyaz renkli, kesit yüzü lobullü, merkezinde kazeifiye alanlar bulunan granulomatöz odaklar gözlendi. Hastadan alınan marazi maddeler (akciğer, karaciğer, böbrek ve lenf yumrusu) Konya Veteriner Kontrol ve Araştırma Enstitüsü Müdürlüğü’ne gönderildi. Laboratuvarlarda bakteriyolojik incelemede lezyonlu dokulardan Ziehl-Neelsen boyama ile yapılan direkt mikroskopik muayenede Mycobacterium etkenleri izole ve identifiye edildi. Histopatolojik incelemelerde yaygın kazeifikasyon, lenfohistoyositik infiltrasyonla birlikte epiteloid ve langhans tipi dev hücre oluşumu ile fibröz proliferasyon gözlendi. Sonuç olarak; kronik öksürük, solunum güçlüğü ve aşırı zayıflama gibi semptomlar gösteren sığırlarda tüberküloz’un göz önünde bulundurulması gerektiği kanısına varıldı.
Abstract
In Afyonkarahisar district, a 9 year-old Brown Swiss cow treated previously was admitted to the clinic because of the ongoing complaints of chronic cough, mucoprulent nasal discharge and excessive cachexia. According to the clinical findings, its body temperature, respiratoric rate and heart beating frequency were 39.1 ºC, 43/min and 77/min, respectively. Cough, anorexia, excessive cachexia, mucoprulent nasal discharge and dyspnoea were observed in the animal and therefore it was thought likely to be tuberculosis in the animal. The animal was sent to slaughterhouse according to “Regulation against Cattle Tuberculosis” since its allergic tuberculin test was positive. In the slaughtered animal, it was observed that there were granulomatous masses in gray-white colour and the sectional surface was lobular in varying 2 to 5 mm dimension with solid density in parietal pleura inner side of the thorax during the macroscopic examination. Also, there were caseous areas at the centre of the masses. The pathological materials (lung, liver, kidney and lymph) were sent to The Directorate of Konya Veterinary Control and Research Institute. In the laboratorial bacteriological examination, the mycrobacterium agents were isolated and identified by direct microscopic diagnosis through Ziehl-Neelsen staining the tissues with lesion. In histopathological examinations, it was observed that there were the formation of epiteloid and langhans typed giant cell and fibrosis proliferation along with common caseification and lymphohistoyositic infiltration. Consequently, it was concluded that tuberculosis should be taken into consideration in cattle showing the symptoms like chronic cough, dyspnoea and excessive cachexia.
