Özet

Mayıs 1993-Ocak 1998 tarihleri arasında Atatürk Üniversitesi Tıp Fakültesi Kalp ve Damar Cerrahisi Anabilim Dalı'na müracaat eden ve akut arter embolisi tanısı konan 158 hastaya uygulan 161 embolektomi girişimi sonuçları değerlendirildi. Vakaların yaşları 21-71 ortalama 49 olup, 94'ü (%59) erkek, 64'ü (%40) kadın idi. Vakaların geliş süresi 2-40 saat ortalama 11 saat idi. Postoperatif erken ölüm 4 (%2) vakada görüldü. Yaşayan hastalardan 11'ine (%6) annputasyon yapıldı. Mortalite görülen hastaların büyük çoğunluğu myokard infarktüsü geçirmiş hastalardı. 10 (%6) hasta reembolektomiye alındı. 16 (%10) hastada yara enfeksiyonu gelişti ve tedavi ile düzeldi. Operasyonun zamanlaması ve birlikte heparin uygulanmış olması prognozu etkilemektedir. Özellikle gecikmiş vakalarda operasyon esnasında revaskularizasyon sendromundan koruyucu yöntemlerin kullanılması mortalite ve morbititeyi azaltmaktadır.

Abstract

Totaly 161 embolectomy were undertaken for 158 patients who applied to The Clinic of Cardiovascular Surgery Faculty of Medicine Atatürk University between May 1993 January 1998 and for whom acute arterial emboli was diagnosed. Age of the patients ranged from 21 to 72 and the average of their ages was 49, 94 (59%) were male and 64 (40%) were female. The period of time between the beginning of disease and application of patients varied between 2-40 hours with an average of 11 hours. There were early deaths in 4 (2%) patients, two of whom were out of heart related causes. Amputation was done for 11 (6%) patients who were alive. The majority of the patients observed mortality, were the ones who had developed myocardial infarction. Reembolectomy was undertaken for 10 (6%) patients. Timing the operation and carying out heparin treatment affect prognose.The usage of methods preventing from revascularisation syndrome during the operation reduces mortality and morbidity in particulary late interfered cases.
