Özet

Bu çalışmanın amacı, benign prostat hiperplazisi (BPH) nedeniyle prostatektomi yapılan hastalardaki insidental mesane tümörü insidansmı araştırmak ve obstrüksiyon ile olan ilişkisini açıklamaktır. Çalışmaya yaşları 53-90 arasında (ortalama:63,4) olan BPH'lı 160 olgu dahil edilmiştir. Tüm vakalardan açık prostatektomi veya transüretral rezeksiyon yapılırken mesane biyopsileri alındı ve histopatolojik olarak incelendi. Mesane tümörü tesbit edilen ve edilmeyen vakalar arasında obstrüksiyon derecesi istatistiksel olarak karşılaştırıldı. Mesane biopsilerinin sonuçları % 3.12 transizyonel hücreli kanser, % 65.63 kronik nonspesifik sistit, % 0.62 interstisyel sistit, % 0.62 tüberküloz sistit ve % 30 normal mesane mukozası ile uyumlu olarak bulundu. Obstrüksiyonun mesane tümörü gelişiminde rolü olduğu istatistiksel olarak gösterildi. Sonuç olarak primer BPH tedavisi esnasında mukozal hipertrofi olarak düşünülen şüpheli alanlardan random biopsi alınması gerektiğini düşünüyoruz.

Abstract

The purpose of this study is to research incidence of insidental bladder tumour in patients who are undergo prostatectomy due to benign prostatic hyperplasia (BPH) and to explain relationship between bladder tumour and obstruction due to BPH. One hundred and sixty cases with BPH, aged range from 53 to 90 years old (mean:63.4) were included the study. While all patients were undergone open prostatectomy or transurethral resection, bladder biopsies were taken from all patients, and histopathological evaluation were performed. The degree of outlet obstruction were statistically compared to in patients with BPH plus bladder tumour and with only BPH. Histopathological results of bladder biopsies were determined 3.13 % transitional carcinoma, 65.63 % chronic nonspecific cystitis, 0.62 % interstitial cystitis, 0.62 % tuberculous cystitis and 30 % normal bladder mucosa. It was statistically showed that obstruction had a role in development of bladder tumour. The most important results of this study is to emphasize the necessity of taking random biopsy samples from the doubtful regions generally thought to be mucosal hypertrophy by evaluating the bladder mucosa carefully during the primary benign prostatic hyperplasia treatment.
