Derin ven trombozunda antifosfolipid antikorlarının pozitifliği

Özet
Antifosfolipid sendromu derin ven trombozu nedenleri arasında önemli bir yer tutmaktadır. Bu çalışmada majör risk faktörü bulunmayan derin ven tromboz (DVT) tanısı almış hastalarda antikardiyolipin antikor (AKA) IgG ve IgM izotipleri ile lupus antikoagulanı (LA) pozitiflik oranı araştırıldı. Çalışmaya major risk faktörü bulunmayan DVT tanısı almış 30 kişiden oluşan hasta grubu ile sağlıklı 20 kişiden oluşan kontrol grubu alındı. Hasta grubunun yaş ortalaması 35,7±4,6 yıl ve kontrol grubunu oluşturan 20 sağlıklı kişinin yaş ortalaması 34,5±6,9 yıl idi. Bu iki grup arasında istatiksel açıdan anlamlı fark yoktu (p>0,05). AKA IgG ve AKA IgM izotipleri ELISA yöntemi, LA aktive parsiyel tromboplastin zamanı ve seyreltilmiş "Russell'viper venom" zamanı (sRVVZ) testleri ile değerlendirildi. AKA IgG izotipi 11 (%37) hastada pozitif, AKA lgM izotipi ve LA tüm hastalarda negatif bulundu, Kontrol grubunda bu antikorların tamamı negatif idi. Major risk faktörü bulunmayan DVT'lu hastalarda antifosfolipid antikorlarının araştırılmasının uygun olacağı sonucuna varıldı.
Abstract
Antiphospholipid syndrome occupies an important place among the reasons of deep vein thrombosis. In the present study we aimed at investigating anticardiolipin antibodies (ACA) IgG, and IgM and lupus anticoagulants (LA) in patients with deep vein thrombosis with no predisposing illnesses. The study included two groups, the patient group of 30 who have had deep vein thrombosis and 20 healthy subjects as control group. The mean age in the patient group was 35,7±4,6 years and this was 34,5±6,9 years in control group. ACA IgG and ACA IgM isotypes were detected by ELISA method and LA by the tests of activated partial thromboplastin time and diluted Russell's venom time. ACA IgG isotype was positive in 11 (37%) patients. ACA IgM isotype and LA were found negative in the patient group. All of these antibodies were negative in control group. It was concluded that these antibodies need to be studied in the patients who have had deep vein thrombosis with no predisposing illnesses.
