Özet
Whipple dışı pankreas cerrahisi yaptığımîz hastalarda morbidité, mortalité, tedavi şekli ve tedavi sonuçlarını değerlendirmek amacıyla Atatürk Üniversitesi Tıp Fakültesi Genel Cerrahi Anabilim Dalfnda Ocak 1991-Mayıs 2001 tarihleri arasında pankreas dokusuna yönelik ameliyat edilen hastaların dosyaları incelendi. Hastaların 18'i erkek, 20'si kadın ve yaş ortalaması 42.1 (11-75 yaş) idi. Tanı için, rutin laboratuar tetkikleri, ultrasonografi (USG), bilgisayarlı tomografi (BT) ve diğer tanı yöntemleri kullanıldı. Kunt ve penetran yaralanma sonucu akut karın ön tanısıyla operasyona alınan 11 hastanın 5'inde pankreas laserasyonu nedeniyle debridman ve hemostaz, 4'üne pankreas kontüzyonu nedeniyle peripankreatik drenaj, 2'sine pankreas kuyruk laserasyonuna bağlı distal pankreatektomi yapıldı. Pankreas kisti perforasyonuna bağlı akut karın ön tanısı ile ameliyata aîman 1 hastaya debridman ve drenaj yapıldı. Pankreatik fistülü olan 2 hastaya Roux-en Y fistülojejunostomi yapıldı. Pankreas psödokisti olan 14 hastanın 11'ine Roux-en Y kistojejunostomi, 2'sine kist aspirasyonu ve drenaj ve 1'ine kistoduodenostomi yapıldı. Pankreasta kitlesi olan 5 hastaya (2 gövde, 3 kuyruk) ve gastrointestinal sistem kanseri olan 4 hastanın 3'üne distal pankreatektomi, diğer hastaya karaciğer tümörünün invaze ettiği pankreas bölgesi rezeksiyonu yapıldı. Bir hastaya insülinoma nedeniyle kitle enükleasypnu yapıldı. Komplikasyon olarak 8 hastada yara yeri enfeksiyonu, 1 hastada eviserasyon, 1 hastada pankreatik fistül gelişti.
Abstract
Thirty-eigth patients who underwent pancreatic surgery other than Whipple procedure, at Department of General Surgery, Atatürk University, School of Medicine Erzurum between January 1991 and May 2001 were reviewed with respect to mortality, morbidity rate, and the result of different treatment modalities. Of the patients, 18 were man and the mean age was 42.1 (11-75 years). Routine laboratory test, USG, CT, endoscopy and other diagnostic procedures were used to confirm the diagnosis. In the five of eleven patients who were operated for blunt and penetran abdominal injuries, pancreatic laceration was observed and debridement and hemostasis were performed. In four of these patients peripancreatic drainage was made for pancreatic contusion and two had distal pancreatectomy for pancreatic laceration. In one patient who was operated under emergent circumtances for pancreatic cyst perforation, depridman and drenage were performed. In two patients with pancreatic fistula, Roux-en Y fistulojejunostomy was performed. Roux-en Y cystojejunostomy was performed in 11 patients, cyst drainage in 2 patients, and cystoduodenostomy in 1 patient who had pancreatic pseudocyst. Distal pancreatectomy was performed in 5 patients with pancreatic mass (2 in bady, 3 in tail) and in 3 patients who had invasion of the pancreas due to gastrointestinal cancer and one patient underwent wedge resection because of invasion of hepatic carcinoma. One patient underwent enucleation for pancreatic insulinoma Infection was seen in 8 patients, evisceration in one, and pancreatic fistula in one patient.
