Çenelik tedavisinde lingual ark uygulamasının dento-alveolar yapılar üzerindekinin etkilerinin incelenmesi

Özet

Bu çalışmanın amacı iskeletsel Sınıf III ano­maliye sahip bireylerin Çenelik ile tedavisinde Lingual ark uygulamasının dento-alveolar yapılar üzerindeki etkilerinin incelenmesidir. Araştırmamızın materyalini, alt çene progna-tisinden kaynaklanan sınıf III anomalisine sahip pubertal büyüme atılımı henüz başlamamış 30 bireyin lateral sefalometrik filmleri oluşturmaktadır. Hastaların 15 tanesine lingual ark ve çenelik uygulanırken, geri kalan 15 hastaya yalnızca çenelik uygulanmıştır. Grup içi değişiklikleri belirlemek için eşleştirilmiş t testi uygulanmıştır. Gruplar arası farklılıkları belirlemek için ise farklar üzerine Student's t testi uygulanmıştır. Her iki uygulamada da büyümenin etkisiyle üst çene öne doğru hareket ederken alt çenenin aşa­ğıya ve arkaya doğru rotasyon yaptığı tespit edilmiştir. İki grubun karşılaştırılmasında ise çeoeıik grubunda L1-NB ve IMPA açılarında azalma olduğu bu azalmanın Ll-NB açısında 0.05 düzeyinde IMPA da ise 0.001 düzeyinde istatistiksel olarak önemli olduğu tespit edilmiştir. Sonuçlar şiddetli olmayan Sınıf III vakalarda lingual arkın dental kompanzasyonu önleyerek istenilen iskeletsel düzeltmenin sağlamasında faydalı olabileceğini göstermiştir.

Abstract

The purpose of this study was to investigate the effects of lingual arch application on the dento-alveolar structures in the chin-cup treatment of individuals who had skeletal class III anomaly. The materials of our research consisted of lateral cephalometric films(taken before and alter the treatment) of 30 subjects who had Class III anomalies growing out of mandibular prognathy and whose pubertal growth had not yet started. Lingual arch and chin-cup were applied to 15 of the patients whereas the other 15 were treated only with chin-cup. To determine the differences within each group paired t test was applied, and students t test was used to determine the differences between the groups. It was observed that, in both applications with the effect of growth maxilla tended to more forward while maodible rotated down and backwards. When the two groups were compared a decrease in the Ll-NB and IMPA angles of chin-cup group was determined, and this decrease was found to be statistically 0.05 significant for Ll-NB angle and 0.001 for IMPA angle. The results indicated that in mild Class III cases, lingual arch can be useful in obtaining the desired skeletal improvement by preventing the dental compensation.
