Özet

Son yıllarda, çeşitli Ortopedik ve Travmatolojik sorunların çözümünde popüler bir yöntem olarak uygulanan Ilizarov tipi sirküler eksternal fiksatörlerin minör komplikasyonların arasında bulunan çivi yolu enfeksiyon oranlarını ve nedenlerini belirleme ve önleme amacıyla, Ilizarov tipi sirküler eksternal fiksatör uygulanan 37'si (% 80.4) erkek ve 9'u (% 19.6) kadın toplam 46 olguda karşılaşılan çivi yolu enfeksiyonları araştırıldı. Ortalama yaş 27.4 (16-71) ve ortalama izlem süresi 5.6 ay (3-11) idi. 46 olgunun 69 kemiğine 1.8 mm kalınlığında toplam 431 K teli uygulandı. Kullanılan 431 K telinden 365'i trokar ve 66'sı süngü uçlu, 348'si düz ve 83'ü zeytinli idi. Teller 25-50 devir/dakika hızla yerleştirildi. Tellerin gerdirilmeleri ayarlı dinamometre yardımıyla yarım halkalarda 50 kg ve tam halkalarda 110-120 kg olacak şekilde yapıldı. Tellerin geçirildiği bölgelerde yumuşak doku sorunu bulunmuyordu. 46 olgudan 22'sinin (% 47.8) toplam 54 (% 12.5) telinde çivi yolu enfeksiyonu saptandı. Çivi yolu enfeksiyonu, 41 (% 9.5) telde yumuşak doku inflamasyonu (grade 1) ve 13 (% 3.0) telde yumuşak doku enfeksiyonu (grade 2) şeklindeydi. Olgularımızın hiçbirinde kemik enfeksiyonuna (grade 3) rastlanmadı. Çivi yolu enfeksiyonlarının ortaya çıkış süreleri 13 (% 24.1) olguda ilk 60 gün ve 41 (% 75.9) olguda ikinci 60 gün içerisindeydi (p=0.0001). Yumuşak doku inflamasyonu ve akıntı gelişen olguların tamamında yapılan mikrobiyolojik araştırmalar sonucunda mikroorganizma üremedi. Yumuşak doku enfeksiyonu olarak değerlendirilen olgularda yapılan mikrobiyolojik araştırmalarda ise 7 (% 53.8) olguda stafilokokkus aureus ve 6 (% 46.2) olguda stafilokokkus epidermidis üretildi. Tüm olgular yapılan çivi giriş yeri pansumanları ve uygun antibiyotik tedavisi sonucunda tamamen iyileşti. Hiçbir olguda osteomyelit gelişmedi. Sonuçta, literatürle uyumlu, tellerin kalış süresi ve çevre yumuşak dokuların fazla olduğu bölgeler ile çivi yolu enfeksiyonları arasında istatistiksel olarak anlamlı ilişki saptandı (p

Abstract

PIN-TRACT INFECTIONS AFTER THE USE OF ILIZAROV TYPE CIRCULAR EXTERNAL FIXATOR Purpose: We planned our study to be based on to determine the rations and defining the causes of pin-tract infections which were enconutered in Ilizarov type circular external fixator applications, used widely as a treatment modality in the orthopaedical and traumatological problems during the last years. Material: We searched the pin-tract infections in 37 male (80.4%) and 9 female (19.6%) patients having fractures treated with Ilizarov type circular fixators with a total of 46 cases. Methods: The mean age was 27.4 months (16-71 months) and mean follow up time was 9.2 months (4-23 months). 431 K wires were introduced to the 69 discrete bones of all 46 cases. Of 431 K wires, 365 of them were trochar ended while 66 of them were bayonet ended, 348 were stright while 83 of them were olived. The wires were implanted with a speed of 25-50 cycle/min and stretcched under 50-120 kg force with the help of a dynamometer. No soft tissue infections were encountered along the way of wire applications. Results: The infections were along the tract of each 54 (12.5%) wires of 22 (47.8%) cases of the total 46. These infections were as soft tissue inflammation (grade 1) in 41 cases and soft tissue infection (grade 2) in 13 cases. None of the cases revealed grade 3 bone infection. The emerging times of the infections at the site of entrance were during the first 60 days in 13 cases; the second 60 days in 41 cases (p=0.0001). No microorganism was cultured with microbiological investigations in the cases of soft tissue inflamation or the cases with dischage. In soft tissue infection cases 7 (53.8%) of them revealed the existency of staphylococcus aureus and 6 (46.2%) of them presented staphylococcus epidermidis. All cases were treated with pin site dressing and proper antibiotherapies. Discussion: Today, external fixarors are being widely used particularly in traumatological problems such as open fractures, multiple closed and comminuted fractures and pseudoarthrosis and in the correction of various contractures and deformities. Besides many advantages, it increments morbidity and mortality by causing complications such as vessel-nerve lacerations and pin-tract infections. In the literature, the insidance of pin-tract infections of Ilizarov have long been given as brief informations in the extists of each study. These rates are well demonstrated in our series as 12.5%. Conclusion: As a results, consistent with the literature, statistically considerable correlations were obtained with wire ends, the residiency time of wires, soft tissue enhanced places and pin tract infections.
