Halluks valgus tedavisinde uyguladığımız proksimal metatarsal kresent osteotomi sonuçlarımız

Özet
Giriş: Bu çalışmada, halluks valgus deformitesi olan hastalarda uyguladığımız proksimal metatarsal kresent osteotomi sonuçlarımızı irdeledik. Hastalar ve Yöntem: Temmuz 1999-Kasım 2002 tarihleri arasında halluks valgus deformitesi olan 14 hastanın 19 ayağına modifiye McBride operasyonu ile birlikte proksimal metatarsal kresent osteotomisi uygulandı. Tümü kadın olan hastalarımızın yaş ortalaması 41 (26-54) idi. Ameliyat sonrası osteotomi hattında radyografik olarak kaynama görülene kadar yürüme alçısı uygulandı ve tespit için kullanılan K telleri 6. haftada çekildi. Ortalama takip süresi 22 ay (5-40 ay) idi. Bulgular: Ameliyat öncesi ortalama halluks valgus açısı 38° (34°-44°) ve intermetatarsal açı 15° (14°-17°) iken bu değerler ameliyat sonrası sırası ile 12° (10°-16°) ve 8° (6°-10°) olarak ölçüldü. Ortalama kaynama süresi 6 hafta olarak saptandı. Son kontrollerinde hastaların AOFAS klinik skorları ortalama 89 (74-100) olarak bulundu ve biri hariç tümü estetik açıdan memnundu. Tartışma: İntermetatarsal açının 14 derece ve üzerinde olduğu halluks valgus olgularında distal yumuşak doku girişimi ile kombine yapılan proksimal metatarsal kresent osteotomisi deformiteyi düzelten etkin bir tekniktir.
Abstract
Purpose: In this study, we evaluated the results of proximal crescentic metatarsal osteotomy performed in patients with for hallux valgus deformity. Patients and Methods: Between July 1999 and November 2002, 14 patients (19 feet) underwent modified McBride procedure and a proximal crescentic metatarsal osteotomy for hallux valgus deformity. All patients were women with a mean age of 41 (range 26-54). A walking cast was applied until the union at the osteotomy site was seen on radiographs, and K-wires used for internal fixation were removed at 6 weeks postoperatively. The mean follow-up was 22 months (5-40 months). Results: The mean preoperative value of the hallux valgus angle was 38° (34°- 44°), and that of the intermetatarsal angle was 15° (14°-17°). The postoperative values averaged 12° (10°-16°) and 8° (6°-10°) respectively. The mean time for union at the osteotomy site was 6 weeks. At the latest follow up visit, the average AOFAS clinical score was 89 (74-100) and all of the patients but one were satisfied with the appearance. Conclusion: Crescentic proximal metatarsal osteotomy combined with distal soft tissue reconstruction is an effective technique in correcting hallux valgus deformity with intermetatarsal angle greater than 14°.
