Segmenter defekt içeren uzun kemik kırıklarının tedavisinde gözenekli hidroksiapatit ve kalsiyum fosfat seramik bloklarının in vivo uyumluluğu

Özet
Amaç: Kemik kaybı ile beraber olan segmenter kırıklar trafik veya iş kazaları gibi yüksek enerjili travmalar sonucu yaygın olarak görülmektedir. Bu durumların bazılarında atrofik kaynama yokluğu gelişmekte ve etkilenen bölgenin mekanik ve biyolojik olarak uyumlu kemik dolgu implantları ile doldurulması gerekmektedir. Hidroksiapatit (HA) bazlı kemik implantların osteokondüktif özelliklere sahip olmalarından dolayı, segmenter kemik kayıplarının tedavisinde kullanılmaları üzerine araştırmalar sürmektedir. Çalışmanın amacı, kemiğe bağlanma kapasitesi yüksek olan trikalsiyum fosfat (TCP) ile HA kompozitinin segmenter kemik kayıplannın iyileşmesi sırasındaki biyouyumluluğunu araştırmaktır. Gereç ve Yöntem: Tavşan tibia segmenter kayıp modelinde kullanılmak üzere %60 HA ve %40 TCP'den oluşan, içinde 100-300 mm çapında mikrogözeneklerin birbirlerine bağlandığı, silindir şeklinde kompozit implantlar üretilmiştir. TCP etkisini karşılaştırmak amacıyla aynı geometri ve mikrodelik yapısına sahip saf HA'dan oluşmuş implantlar kontrol grubu olarak kullanılmıştır. Hazırlanan implantlar 3 aylık tavşan ablalarının 1/3 orta bölgesinde yaratılan l cm uzunluğundaki segmenter kemik kayıplarına yerleştirilmiştir. İmplantiann patolojik uyumluluğu 1. ve 2. ayin sonunda alınan örneklerde incelenmiştir. Sonuçlar: Kullanılan HA-TCP kompozit ve HA implantların yüzeyinde 1. ayın sonunda fibröz kapsülün oluştuğu ve bu kapsülden mikrogözenekli yapının içine doğru bağ dokusunun ilerlediği saptanmıştır. İki ay sonra bağ dokusunun yerini yeni kemik dokusuna bıraktığı gözlenmiştir. Histolojik incelemede yabancı cisim doku yanıtının belirleyicisi olan fagositlere rastlanmamıştır.Çıkarımlar: Yapılan in vivo çalışmalann sonucunda HA ve HA-TCP seramiklerin doku uyumluluğu saptanmış olup bunların uzun kemik segmenter defektlerinin iyileşmesinde güvenle kullanılabileceği gösterilmiştir.
Abstract
Purpose: Segmental bone loss is frequently seen after high energy trauma. In such cases, atrophie non-union occurs and there is a need for mechanical and biological restoration of the defect location. Bone implants composed of hydroxyapatite (HA) and tricalcium phosphate (TCP) have good osteoconductive properties and their use in segmental defect healing is under investigation. The aim of this study was to search the biocompatibility of porous ceramic implants of HA and TCP in segmental defect healing. Material and Methods: Composite ceramics implants of 60 %HA and 40 %TCP consisting of interconnected porous structure were produced in cylindrical shapes to be used in the rabbit tibiae. Same-structured pure HA implants were also produced as for control. Prepared ceramic implants were placed into a 1.0 cm long segmental defect created in the mid 1/3 of the rabbit tibia. Fixation was provided by an intramedullary K-wire only. Compatibility of implants was histologically evaluated through ceramic samples collected at the end of one and two months.Results: Fibrous encapsulation was observed in both composite and HA ceramics. In one month, connective tissue ingrowth into the pores of the implants was observed. At the end of the second month, the connective tissue was completely replaced with woven bone tissue. Phagocytes were not observed in any time point. Discussion: In the early stages of the in vivo experiments, HA and TCP ceramics presented excellent biocompatibility. In conclusion, both types of implants were observed to have great potential for the healing of segmental defects in long bones.
