Karpal tünel sendromlu hastalarda beden kitle indeksi ve şiddetin değerlendirilmesi

Özet
Giriş: Çalışmamızda başta obezite olmak üzere bazı kişisel risk faktörlerinin Karpal Tünel Sendromu (KTS) gelişimindeki rolünü ve obezitenin KTS şiddeti üzerindeki etkilerini araştırdık. Hastalar ve Yöntem: Anamnez, fizik muayene ve EMG sonrasında KTS tanısı konan 55 hastanın 80 eli çalışmaya dahil edildi. Beden Kitle İndeksi (BKİ) (kg/$m^2$) hesaplandıktan sonra hastalar WHO kriterlerine göre gruplandı. KTS'nun şiddeti ise duysal ileti hızı (DİH) ile değerlendirildi. Bulgular: Sekiz (%14,5) hastada hipotiroidi, 6 (%10,9) hastada geçirilmiş radius distal uç kıngı tespit edildi. Yirmi hastada 5- 40 adet/gün arasında değişen en az 10 yıllık sigara kullanımı öyküsü vardı. Beş hastamızın geçmişte daktilograf olarak, l hastamızın ise bilgisayar operatörü olarak halen aktif olarak çalışmaya devam ettiğini öyküsünü aldık. Hastaların %3,6'sı zayıf, %14,5'i normal kilolu, %25,5'i fazla kilolu, %47,3'ü obez ve %9,1'i morbid obez idi. Ortalama BKİ 30,7 kg/$m^2$ (18-42) olup 45 (%81,9) hasta fazla kilolu, obez ve morbid obez grubundadır. KTS'nun şiddetine göre sınıflandığında 80 elden 34 (%42,5) şiddetli, 24 (%30) orta, 22 (%27,5) ılımlı olarak değerlendirildi. Tartışma: Yüksek BKİ olan hastalarda n.medianus'un sıkışmasına sinir etrafında artmış olan yağ dokusunun zemin hazırladığını düşünüyoruz. İdiopatik KTS multifaktöryel bir durum olup kişisel risk faktörlerinden hiçbirinin tek başına sorumlu olamayacağı, obezitenin ise KTS gelişiminde önemli bir risk faktörü olduğu kanısındayız.
Abstract
Introduction: The purpose of the study is to evaluate predisposing risk factors majorly obesity in Carpal Tunnel Syndrome (CTS) and effects of obesity on the severity of CTS. Patients and Method: Eighty median nerve entrapment around wrist in fifty-five patients diagnosed with history, physical examination and, EMG analysis were included in the study. Patients were gouped according to WHO criterias after Body Mass Indexes (BMI) were calculated. Sensorial conduction velocity is used for detecting severity of the disease. Results: Eight (12,9%) had hypothyroid disease, 6 (10,9%) had distal radius fracture. Minimum 10 year history of 5 to 40 cigarette/day smoking were recorded in 20 patients. Five patients were typist in their decent working period and 1 was still working as a PC operator. The patients were grouped as 3,6% low weight, 14,5% normal weight, 25,5% over-weight, 43,7% obese and, 9,1% morbid obese. Mean BMI was 30,7 kg/$m^2$ (18-42). Fourty five (81,9%) paitents were in the group of over-weight, obese and, morbid obese. Discussion: We believe there is predisposition in nerve entrapment with possible increased fatty deposition arround median nerve in patients with high BMI. However, CTS is multifactorial, obesity should be regarded as a major risk for CTS.
