Travmatik Doğum Eylemi ve Ebelik Bakımı

Özet
Travmatik doğum, kadının doğurganlık sürecinin herhangi bir aşamasında ortaya çıkan doğurma eylemini, doğacak olan bebeği ya da kendisinin fiziksel bütünlüğünü tehdit edecek bir durumun ortaya çıkması ya da anne tarafından bu şekilde algılanmasıdır. Travmatik doğum deneyimine sahip olan kadınlar, doğum anını; çaresizlik, kontrolünü kaybetme hissi, kuvvetli bir korku ve dehşet anı olarak tanımlamaktadır. Ayrıca bu kadınlar olayın sık sık hatırlanması, olayla ilgili gece kâbuslar görülmesi ve olayın yeniden yaşantılanması gibi posttravmatik stres semptomları gösterebilmektedirler. Annelerin doğum eylemine yönelik algıları, doğum sürecine ve bu süreçte yaşadıkları olaylara bakış açıları ve bunlara yükledikleri anlamlar farklılık gösterebilir. Buradan yola çıkarak kadınların yaşamında aslında normal, sağlıklı, fizyolojik ve doğal bir süreç olması gereken doğumun travmatik olarak algılanmaması için prekonsepsiyonel ve interkonsepsiyonel dönemlerde, prenatal ve postnatal izlemlerde ya da doğumhane ve obstetrik servislerde bulunan riskli kadınların tanılanabilmesi ve erken süreçte, gerekli nitelikli ebelik bakımının uygulanması oldukça önemlidir. Bu derlemenin amacı, travmatik doğum sürecini, kadının bu süreçte ki deneyimlerini ve gerekli ebelik bakımını içeren bilgilerle litratüre katkı sunmaktır.
Abstract
Traumatic birth is the occurrence of a situation that will threaten the baby to be born or the physical integrity of the woman at any stage of the fertility process or the act of giving birth, or the perception of it by the mother in this way. Women who have had a traumatic birth experience, describe the moment of birth; helplessness, the feeling of losing control, a strong fear and horror moment. In addition, these women may show posttraumatic stress symptoms such as frequent recollection of the event, having nightmares about the event, and re-experiencing the event. Mothers' perceptions of labor, their perspectives on the birth process and the events they experience in this process, and the meanings they attribute to them may differ. From this point of view, it is important to identify risky women in preconceptional and prenatal periods so that birth, which should be a normal, healthy, physiological and natural process in women's lives, is not perceived as traumatic. Risky women who are in interconceptional and postnatal follow-ups or in delivery rooms and obstetric services need to be diagnosed early and receive qualified care. The purpose of this review is to emphasize the importance of giving the necessary care with the traumatic birth process and the experiences of the woman in this process.
