Kronik böbrek yetmezliği belirtileri gösteren kedi ve köpeklerde klinik, laboratuvar ve patolojik bugular

Özet
Bu çalışma, klinik ve laboratuar muayenelerde kronik böbrek yetmezliği (KBY) belirtileri bulunan orta-erişkin yaşlı 7 köpek ve 11 kedi ile klinik olarak sağlıklı orta-ileri yaşlarda 5 kedi ve 5 köpekte yapıldı. Klinik olarak tüylerde karışıklık ve matlık, mukozalarda kuruluk ve solgunluk, dehidratasyon, aşırı zayıflık, yürümede isteksizlik, bitkinlik ve bazısında premolar-molar dişler hizasında kızarıklık ve ülsere kadar varan lezyonlar ile nefeste pis koku saptandı. Hematolojik muayenede retikulosit sayısı, ve MCV'de artış, hematokrit ve eritrosit sayısında azalma, lenfosit ve monosit yüzdelerinde artış, nötroflllerde sağa kayma vardı. Üre ve kreatinin değerlen artmıştı, idrarın mikrobiyolojik muayenesinde Escherichia coli, Streptococcus spp., Leptospira spp. ve Klebsiella spp., fiziksel muayenede koku, yoğun albuminuri (+++++), hematuri ve dansitede azalma (1007-1016), mikroskobik kontrollerde ise, her sahada ortalama 15-20 eritrosit, 10-20 lökosit, çok sayıda epitel döküntüsü, 10-15 yağ globuli, 7-8 adet triple fosfat kristali ve müköz iplikçikler saptandı. Elektrokardiyografide sinüs aritmi, P dalgasında genişleme, P-R aralığında uzama ve kalp vurum sayısında artış belirlendi. Ultrasonografide böbrek konturlarımn bozulduğu, korteks ve medullada hiperekoik ve anekoik alanların varlığı gözlendi. Böbreklerin patolojik incelemesinde, subakut veya kronik glomerulonefritis, fokal dissémine intersitisyel nefritis, tubulonefroz ya da tubullerde dejeneratif değişikliklerle karakterize nefrotik değişiklikler ve birer olguda böbrek kisti, hidronefroz ve primer tümör tanısına ulaşıldı. Patolojik bulgular açısından kedi ve köpek böbrekleri arasında farklılık görülmedi. Böbrek bulgularının yanı sıra diğer organlarda da patolojik bulgular dikkati çekti.
Abstract
In this study, middle-aged 11 cats and 7 dogs with chronic renal failure (CRF) and clinically healthy 5 cats and 5 dogs were used. Clinical examination showed unkempt hair coat and dirty skin, dry and sticky mucous membranes, dehydration, weakness, reluctancy in walking, tiredness, oral ulcérations around premolar and molar teeth and halitosis. In haemotologic examination, increase in the number of reticulocytes and mean corpuscular volume (MCV), decrease in the PCV and erythrocyte count and increase in the percentage of the lymphocytes and neutrophils and the serum urea and creatinin values were found. A degenerative right shift was seen in the neutrophils. Escherichia coli, Streptococci spp., Leptospira spp., and Klebsiella spp. were detected in the microbiological examinations of the urine collected by cystocentesis. The foul-smelling urine, significant proteinuria (+++++) and haematuria, and decreased urine specific gravity (1007-1016) were observed in the physical examination. In the microscopic examinations of the urine sediment 15-20 erythrocytes, 10-20 leucocytes, abundant epithelial debris, 10-15 fatty globules, 7-8 triple phosphate crystals, and mucous fibrils were detected per microscopic field with mean rates. At electrocardiography, sinus arrhythmia, increase in width or height of the P-wave, prolongation of the P-wave and P-R interval were determined. Mean rate was also increased. On ultrasonography, irregular kidneys with hipoechogenic or hiperechogenic cortex and medulla were observed. In the pathological examination of the kidneys, glomerulonephritis, interstitial nephritis, nephrotic changes, cyst, hydronephrosis, and a primary tumor were diagnosed. It was not observed differences between the pathological findings of the kidneys of cats and dogs. It was also found other visceral organ findings besides kidneys.
