Özet

Allojeneik periferik kök hücre transplantasyonu (AlloPKHT) sonrasında graft versus host hastalığı (GVHH) allojeneik kemik iliği transplantasyonuna (AlloKlT) oranla daha sık görülmektedir. Heparinin deney hayvanlarında bilinmeyen bir mekanizma ile GVHH önlediği gösterilmiştir. Bu çalışmada kök hücre kaynağı veya profilaktik düşük molekül ağırlıklı heparin (DMAH) kullanımına göre, 1)Th1 (TNF-alfa, IFN-gama)/Th2 sitokinlerin (IL-10) dengesini değerlendirmeyi ve 2) Solubl adezyon moleküllerinin (SAM) (sVCAM, sICAM, sL-selektin) düzeylerini izlemeyi amaçladık. Çeşitli hematolojik maliniteleri olan 30 hasta profilaktik olarak DMAH alan ve almayan olarak prospektif randomize edildi. Kan örnekleri -9.gün, infüzyon günü, aplazi ve engrafman gününde alındı. Sitokin ve SAM düzeyleri .ELISA yöntemiyle çalışıldı. Tüm olgular birlikte değerlendirildiğinde TNF-alfa, IFN-gama ve slCAM-1'in haftalık seyirleri değişiklik göstermeden seyrederken, IL-10, sVCAM-1 ve sL-Selektin düzeyleri hazırlama rejimi ile azalıp, engrafmanla arttı. AlloPKHT ve AlloKlT yapılan olgularda sitokin ve SAM düzeylerinin seyri, AlloPKHT grubunda hazırlama rejimi sonrası azalan ve engrafmanı takiben artan IL-10 düzeyi dışında, benzerdi. DMAH alan ve almayan hastalar arasında sitokin ve SAM düzeyleri arasında anlamlı farklılık saptanmadı. IL-10 düzeyleri akut AGVHH şiddeti ile pozitif korelasyon gösterdi (r=0,517, p= 0,049) Sonuçta, ne kök hücre kaynağı ne de DMAH profilaksisinin Th 1/Th2 sitokinleri ve SAM'e üzerine bir etkisi olmadı. Bazal sitokin veya SAM düzeyleri allojeneik transplantasyon sonrası görülen akut GVHH gibi komplikasyonlarda belirleyici olmamasına rağmen, IL-10 dü-zeyleri ile akut GVHH'nin şiddeti arasında pozitif ilişki saptandı.

Abstract

A higher incidence of graft-versus-host disease (GVHD) has been observed at allogeneic peripheral stem cell transplantation (AlloPBSCT) in comparison to allogeneic bone marrow transplantation (AlloBMT). It has been demonstrated that heparin prevented GVH reaction in animal models by an unknown mechanism. In this study we intended: 1) to evaluate the balance between the levels of Th1 TNF$alpha$ and IFN$beta$)/Th2 cytokines(ILIO) and 2) to follow the levels of soluble adhesion molecules (SAM) (sVCAM, sICAM, sL-selectin) according to the stem cell source or the use of low molecular weight heparin (LMWH) for prophylaxis. Thirty patients with various hematological malignancies were randomized to use of DMWH or not. Blood samples were obtained on day -9, infusion day, aplasia period and engrafment day. Cytokines and SAM levels were measured by ELISA method. In the group as whole, weekly levels of TNF_IFN_and sVCAM remained stable and increased following engraftment. The serial measurements of cytokines and SAM levels with the exception of IL10 levels, which decreased after conditioning regimen and increased during engrafment in the AlloPBSCT group, were similar in patients who underwent AlloPBSCT or AlloBMT. There were significant differences in the serum levels of cytokines and SAMs between the patients who received LMWH and who did not. IL10 levels showed positive correlation with severity of acute GVHD. In conlusion, neither the stem cell source nor the LMWH prophylaxis had any effect on Th1/Th2 type cytokines and SAM. Although baseline cytokines or SAM levels could not predict complications seen after allogeneic transplantation such as acute GVHD, IL10 levels correlated with the severity of acute GVHD.
