Özet

Çocuklarda sık görülen toplumdan edinilen pnömoniler (TEP) önemli bir morbidite ve mortalite nedenidir. Etkenin saptanmasmdaki güçlüklerden dolayı genellikle bu enfeksiyonların tedavisi başlangıçta ampirikdir. Sefuroksim sık görülen alt solunum yolu enfeksiyonlarına karşı etkili bir ikinci kuşak sefalosporindir. Parenteral ve oral olarak kullanılabilmesi ardışık tedavide kullanılmasını sağlamaktadır. Toplumdan edinilmiş pnömonisi olan 32 çocuk hastada sefuroksim, sefuroksim aksetil tedavisinin etkinliği, güvenilirliği ve tolere edilebilirliği araştırıldı. Tedavi sonrası yapılan değerlendirmede 29 hastada (%90,7) tam iyileşme, 2 hastada (%6,2) kismi iyileşme ve bir hastada (%3,1) tedavi başarısızlığı saptandı. Otuz iki hastadan üçünde (%9,6) tedaviyi sonlandırmayı gerektirmeyen yan etkiler görüldü. Sonuçta, çocuklardaki TEP için sefuroksim ile ardışık tedavinin etkin, güvenilir, iyi tolere edilen ve tedavi maliyetini azaltan bir tedavi yöntemi olduğu kanısına varıldı.

Abstract

Community-acquired pneumonia (CAP) is a common illness that results in substantial morbidity and mortality in children. Because of difficulties in identification of the pathogen responsible for infection, initial treatment is commonly empiric. Cefuroxime, a second-generation cephalosporin, is active against the most common pathogens associated with CAP. It is available in both parenteral and oral formulations, allowing use in sequential therapy. This study investigated the efficacy, safety and tolerability of a sequential therapy regimen with intravenous cefuroxime followed by oral cefuroxi-me axetil for the treatment of CAP in 32 children. Post-treatment evaluation showed a complete re-covery in 29 (90,7%) patients, partial recovery in two patients (6,2%) and treatment failure in one patient (3,1 %). Adverse events, which did not require the discontinuation of therapy, were observed in three (9,6%) out of 32 patients. Sequential therapy with cefuroxime followed by cefuroxime axetil for the treatment of CAP in children is an efficacious, safe, well-tolerated and cost-saving treatment regime.
