Özet

Tiroglossal duktus kistleri, tiroglossal duktusun kısmi veya tam obliterasyon eksikliği ve içindeki epitelin sekretuar özelliği sonucunda gelişen benign kistik oluşumlardır. Çoğunlukla boyun orta hattında, yutkunma sırasında vertikal doğrultuda hareketli ve ağrısız bir şişlik olarak ortaya çıkan bu lezyonlar bazen cilde fi stülize olabilirler ve tiroglossal fi stül olarak adlandırılırlar. Bu makalede kliniğimizde tiroglossal duktus kisti ve/veya fi stülü tanısı ile Sistrunk ameliyatı yapılan 21’i primer, 4’ü nüks olgu olmak üzere toplam 25 olgu sunuldu. Tüm olgular ayrıntılı anamnez, KBB muayenesi, boyun USG ve/veya CT ile değerlendirildi. Olguların 17’sinde fi stül (% 68), 8’inde (% 32) kist formasyonu saptandı. Postoperatif dönemde 2 hastada antibiyotik tedavisine yanıt veren yara enfeksiyonu gelişti. Ameliyat edilen hastaların ortalama 32 aylık takiplerinde nüks görülmedi. Tiroglossal duktus kistlerinde malign dejenerasyon riski olduğu için cerrahi olarak tedavi edilmelidir. Tedavide Sistrunk ameliyatı en az rekürrens oranıyla güvenli ve komplikasyon riski düşük bir cerrahi prosedürdür.

Abstract

Thyroglossal duct cyst are benign cystic formations those develop due to total or partial obliteration defect of thyroglossal duct with secretuary function of the surrounding epithelium. They are usually located at the midline of the neck and move vertically during swallowing. They are usually painless and sometimes they can fi stulae to skin and termed as thyroglossal fi stulae. In this study we present 25 cases diagnosed as thyroglossal duct cyst and/or fi stulae in our clinic. 21 out of these cases had primary, 4 had recurrent disease and all are undergone Sistrunk operation. All the cases were evaluated by detailed history, otorhinolaryngologic examination, neck ultrasonography or computed tomography. 17 of the cases (68%) were observed to have fi stulae and 8 (32%) cyst formation. In 2 patients postoperative wound infection was developed and responded to antibiotic therapy. No recurrence was observed in 32 months follow up period. Thyroglossal cysts have to be operated since there is risk of malign degeneration. Sistrunk operation has the lowest recurrence rate and it is a reliable method with a low complication rate.
