Özet

Sifiliz, Treponema Pallidum'un neden ol¬duğu, birçok deri ve müköz membran lezyonla-n ile ortaya çıkan kronik ve spesifik bir infeksiyon hastalığıdır. Oral lezyonlar hastalığın üç aşamasından herhangi birinde ortaya çıkabilir. Şankr ya da sifilizin primer lezyonu, spiroketlerin inoküle olduğu yerde gelişir ve enfeksiyonun bulaşmasından itibaren genellikle 2-6 haftalık bir inkubasyon süresi vardır. İkinci dönem sifilizde görülen lezyonlar deri lezyonlan veya plak müközler şeklinde ortaya çıkmaktadırlar. Plak müköz lezyonlan sifilizin bulaşma riskinin en fazla olduğu lezyonlardır ve yoğun spiroket içerirler. Bu çalışma 1995 yılında Gülhane Askeri Tıp Akademisi Dişhekimliği ve Dermatoloji Polikliniklerine müracat eden ikinci dönem sifilizli 16 hastayı kapsamaktadır. Yaş ortalamalan 20.8 olan hastalann 15'i erkek, l'i kadındı. Klinik muayenede on hastada plak müköz lezyonlan olduğu belirlendi. VDRL testi hastalarda 1:6-1:32 oranlan arasında pozitif bulundu. Sifilizde plak müköz lezyonlannın çok bulaşıcı olması nedeniyle Dişhekimleri muayene ve ayırıcı tanıda çok dikkatli olmalıdır.

Abstract

Syphilis is a chronic, specific infection caused by the spirochete Treponema Pallidum, which results in many cutaneous and mucous membrane manifestations. Oral lesions can occur in any of three stages of the disease. The chancre, or primer lesion of syphilis develops at the point of inoculation, usually in a period of 2 to 6 weeks after the enterance of the spirochete. The secondary stage is characterized by cutaneous eruption and mucous patches in the oral cavity. The mucous patch is the most contagious lesion of syphilis, because its surface is covered with an abundance of spirochetes. The study consisted of sixteen patient with secondary syphilis admitted to the Department of Dentistry and Dermatology of Giilhane Military Medical Academy in 1995. Mean age of patients were 20.8 years (1 female, 15 male). On clinical examination, we observed mucous patches on ten of the patients. The VDRL test were active in titer of 1:6-1:32 in the patients. Because of the mucous patches are most contagious lesions of syphilis, dentists chould be more careful on the dental examination and differential diagnosis.
