Özet

Komplike mandibula fraktürlerinin teda¬visi oral cerrah için önemli bir soran olmaktadır. Mandibula fraktürlerinin tedavisi, fragmanların uygun repozisyonunun ve rijit fıksasyonunun elde edilmesi ile en iyi şekilde sağlanmaktadır. Son yıllarda miniplaklar birçok avantajlarının bulunması nedeniyle, mandibula fraktürlerinin tedavisinde ilk seçenek olarak kullanılmaktadır. Mandibula fraktürlerinin tedavisi sonrası gelişen kemik v.e yumuşak doku infeksiyonu en sık gö¬rülen kompldkasyonlardan biridir. Özellikle cer¬rahi girişim öncesi infekte mandibula fraktür-lerinde miniplaklarm kullanımı tartışmalıdır. Bu makalede, tel osteosentezi ile tedavisi yapılmış bir , mandibula fraktürü olgusunun, sonradan gelişen osteomyelit nedeniyle antibi¬yotik tedavisi, küretaj ve miniplak osteosentezi kombinasyonu kullanılarak yeniden tedavisi sunulmaktadır.

Abstract

The treatment of the complex mandibular fractures is a challenge for the oral surgeon. The healing of mandibular fractures is best maintained by adequate repositioning of the fragments and rigidity of the fixation. Recently miniplates have been the first choice for the treatment of mandibular fractures by their many advantages. After the treatment of mandibular fractures, infection of bone and soft tissues is one of the most frequently seen complications. Especially the use of miniplates in preoperatively infected mandibular fractures is a controversial subject. In this article a case which was treated by the combination of antibiotic therapy, curettage and miniplate osteosynthesis, because of the osteomyelitis which had developed following the previous wire osteosynthesis is presented.
