Özet

Periodontal dokular ve diş pulpası anatomik ve fonksiyonel olarak birbiriyle ilişkide olan yapılardır. Bu nedenle periodontal ve endodontik problemler ayrı olarak gelişse bile zamanla kombine tedavi yaklaşımlarını gerektirecek şekilde, meydana gelen patoloji, hem dişi hem de çevre dokularını içerecek bir yayılma gösterebilir. Çok köklü dişlerde izlenen bu tıp kombine lezyonlarda prognoz, uygun bir tedavi seçeneği yoksa çoğunlukla beklenildiği kadar ümit verici değildir. Son yıllarda periodontal rejenerasyonu destekleyen yöntem ve materyallerin, çok köklü dişlerde gelişen lezyonlarda kullanımı ile ilgili başarılı sonuçlar bildirilse de teknik prensipleri ile ilgili ve ekonomik dezavantajları hala tartışılmaktadır. Bu makalede. periodontal-endodontik sorunlu çok köklü dişlerde, konvansiyonel cerrahi tekniklerinden olan ampütasyonunun farklı endikasyonları irdelenmiş ve 3 olguda uzun dönem klinik ve radyografik bulguları ortaya konulmuştur. Sonuç olarak, kökleri ampute edilmiş dişlerin prognozunun daha önceden inanıldığı gibi ümitsiz olmadığı fakat destekleyici periodontal tedavi safhasında düzenli kontrollerin gerekli olduğu düşüncesindeyiz.

Abstract

The periodontal tissues and dental pulp are related with each other closely both anatomically and functionally, therefore, although the diseases of these tissues are considered as different entities, the pathology may demonstrate a progression that requires a combined therapy approach. The prognosis of multi-rooted teeth with such combined lesions is thought to be hopeless if appropriate therapy would not be applied. Recently, the use of regenerative techniques and materials for the treatment of the teeth with furcation involvement improve the outcomes of therapy but advantages and disadvantages with regard to the technical principles and the cost of regenerative materials still remain as a question. In this report, the indications of root amputation procedure, a conventional periodontal surgery technique, in maxiller molar teeth with periodontal-endodontic infection are discussed and the long-term evaluation of three cases both clinically and radiographically are also presented. As a conclusion, the prognosis for root-resected molars may not be as hopeless as previously believed but regular réévaluations during the maintenance period are necessary.
