Kompozit restorasyonların yapımından önce ve sonra uygulanan vital ağartma işlemlerinin mikrosızıntı üzerine etkileri

Özet
Bu in vitro çalışmada, kompozit restorasyonların yapımından önce ve sonra uygulanan vital ağartma işlemlerinin mikrosızıntı üzerine etkilerinin incelenmesi amaçlanmıştır. Bu amaç için; protetik ve periodontal nedenlerle çekilen 15 adet çürüksüz insan daimi molar dişi kullanıldı. Dişler rastgele 3 gruba ayrıldı. Birinci grupta, her dişin bukkal ve ligual yüzeylerine sınırları minede ve 3 mm derinliğinde Black Class V kaviteler açıldı. Kaviteler daha sonra kompozit rezin ile üretici firmaların önerilerine uygun şekilde restore edildi. Bu grup kontrol grubu olarak değerlendirildi. İkinci grupta, her dişin bukkal ve lingual yüzeylerine 9 gün 2 saat süreyle % 35'lik hidrojen peroksit ağartma ajanı uygulandı. Daha sonra, bu dişlere kontrol grubunda olduğu gibi kaviteler açıldı ve kompozit rezin ile restore edildi. Üçüncü grupta ise, dişler ilk olarak kontrol grubunda olduğu gibi restore edildi daha sonra dişlerin bukkal ve lingual yüzeylerine ikinci grupta olduğu gibi ağartma ajanı uygulandı. Deneyler süresince tüm örnekler 37 °C de distile su içinde bekletildi. Daha sonra örneklere 5 2 C'de 1 dakika 55 2 C°'de 1 dakika olmak üzere 200 kez termal siklus uygulandı. Bu işlemden sonra dişlerin kök uçları kompozü dolgu maddesi ile kapatılıp restorasyon sınırlarının 2 mm. dışında kalan tüm kısımlar iki kat tırnak cilası ile boyandı, ardından %0.5 'lik bazik fuksin solüsyonunda 24 saat bekletildi. Dişler boyadan çıkarıldıktan sonra bukko-lingual yönde ikiye ayrıldı. Elde edilen kesitlerin mikrosızıntı farklarının değerlendirilebilmesi için fotoğraftan alındı. Gruplar arası mikrosızıntı farklılıkları istatistiksel olarak "Ki-Kare" testi ile değerlendirildi. Bu değerlendirmeler sonucu test edilen gruplar arasında ve grupların kendi içlerindeki oklüzal ve gingival kavite kenarları arasında oluşan sızıntı farklılıklarının istatistiksel olarak önemli olmadığı bulgulandı. Yapılan deneyler, kompozü restorasyon yapımından önce veya sonra uygulanan vital ağartma işlemlerinin mikrosızıntının artmasına neden olduğunu fakat bunun istatistiksel olarak önemli olmadığını gösterdi.
Abstract
The aim of this in vitro study was to evaluate effects of pre-operative and post-operative vital bleaching technique on microleakage of composite restorations. For this aim, 15 permanent human molar teeth without caries, extracted for periodontal and prosthetic reasons were used. Teeth were randomly divided into three equal subgroups. At the first group, Black Class V cavities in confinement of enamel and 3mm in depth were prepared on both buccal and lingual face of each tooth. They were restored with composite resin according to the manufacturer directions. This group was served as a control group. At the second group, buccal and lingual face of each tooth was exposed with %35 hydrogen peroxide bleaching agent for two hours periods per day for 9 days. Then teeth were prepared and restored with composite resin as a same manner in the control group. At the third group, the teeth were prepared and restored as a control group then bleaching agent was applied on to the teeth as a second group. All specimens were stored in distilled water at 370 C during treatment periods. Then, 200 x thermocycling was applied between 5 2 for one minute and 55 2 for one minute. After this procedure the apices each of the teeth were covered with chemically cured composite resin and each specimens was sealed with two layers of nail varnish leaving 2 mm. of space around the restorations. The teeth were held in %0.5 basic fucsin solution f or 24 hours and sectioned in to the two parts, bucco-lingual direction. The specimens were then evaluated for staining patterns under stereomicroscope and photographs were taken. Microleakage scores were evaluated statistically by using "Khi-Square" test. There were no statistically significant differences between groups and occlusal-gingival cavities margins of each tooth. Our results showed that pre-operative and post-operative bleaching treatment adversely affected the marginal seal of composite resin. But this affect was not statistically important.
