Özet

Endodontik tedavili dişlerin kırılma olasılıkları, kron ve kök dentinindeki kaybın miktarı ile orantılı olarak artar ve bu durum daimi restorasyonun tutuculuğunda sorunlara neden olabilir. Dolayısıyla bu tür dişlerin protetik restorasyonlarda kullanılması, geriye kalan diş yapısının güçlendirilmesi ile seçilecek olan post-core restorasyonunun yeterli tutuculuk ve dirence sahip olmasını gerektirir. Son yıllarda amalgam restorasyonlarla birlikte adeziv sistemlerin kullanımının amalgamın kırılmaya karşı direncini arttırdığı ileri sürülmektedir. Bu çalışmada, dentin üzerine uygulanan amalgam bağlayıcı ajanın, prefabrik post kullanılan ve kullanılmayan amalgam core'un kırılma direnci üzerindeki etkisi araştırılmıştır. Akrilik bloklara gömülen 28 adet çekilmiş mandibular premolar dişin kron kısımları servikal bölgelerinden kesilerek uzaklaştırıldıktan sonra, dişler dört gruba ayrılmıştır. Birinci grupta core şekillendirilmesi için sadece amalgam kullanılırken, ikinci grupta amalgam restorasyon öncesinde kök yüzeyine adeziv uygulanmıştır. Üçüncü grupta amalgam core restorasyonu post ile desteklenmiş, dördüncü grupta ise post uygulamasına ilaveten kök yüzeyine adeziv sürüldükten sonra amalgam core oluşturulmuştur, örnekler daha sonra Llyod test cihazında, 1 mm/dak'da kırılana kadar yüklenmiş ve sonuçlar Kruskal-Wallis yöntemiyle değerlendirilmiştir. Gruplar arasında istatistiksel olarak anlamlı bir farklılık bulunmamasına rağmen, en yüksek ortalama bağlantı direnci değeri ikinci grupta görülmüştür.

Abstract

The possibility of fracture of endodontically treated teeth increases depending on the amount of the crown loss and root dentin structure and this may result with problems about the retention of the permenant restoration. Therefore the use of these teeth for prosthetic restorations requires the inforcement of the remaining tooth structure and the sufficient retention and strength of the post-core restoration. Recently it has been reported that the use of the adhesive systems with amalgam restorations increases the fracture resistance of the restorative material. In this study, the influence of bonding agent on the fracture strength of amalgam core restorations with or without prefabricated post application was evaluated. The crowns of 28 mandibular molar teeth embedded in acrylic resin blocks were sectioned from their cervical regions and the teeth were divided into four groups. While only amalgam was used for the fabrication of the cores for the first group, an adhesive was applied on root dentin surface before the core build-up in the second group. In the third group the core restoration was supported by the prefabricated post and the adhesive was applied in addition to post before the core fabrication. The samples were loaded on the Llyod test machine under 1 mm/min until core fracture occured and the results were statistically analysed by Kruskal-Wallis test. Although no statistically significant difference was found among the groups, the highest mean shear bond strength was observed in the forth group.
