BİR AİLE HEKİMLİĞİ BİRİMİNDEKİ OKUL ÇAĞIİZLEMLERİNİN RETROSPEKTİF ANALİZİ

Özet
Amaç: Çocukların büyüme ve gelişmelerinin takibi önemli bir konudur. Nitekim aile sağlığı merkezlerinde ilgili protokol ve mevzuatlarda öğrencilerin aile hekimleri tarafından her yıl periyodik muayene /izlemlerinin yapılması gerektiği belirtilmektedir. Bu çalışmada, öğrenci izlemlerini değerlendirmek amacıyla Okulda Sağlığın Korunması ve Geliştirilmesi Programı’nın uygulandığı bir aile sağlığı birimindeki okul çağı izlemlerinin sonuçları analiz edilmiştir. Materyal ve Metot: Çalışmamız retrospektif tanımlayıcı tipte bir gözlemsel çalışmadır. Ankara Keçiören’de bir aile hekimliği birimine 1 Ocak - 1 Haziran 2019 tarihleri arasında “Okulda Sağlığın Korunması ve Geliştirilmesi Programı” kapsamında okullardan yönlendirilerek başvuru yapan tüm öğrencilerin izlem verileri ilgili aile hekimliği bilgi sistemi (AHBS) üzerinden derlenerek değerlendirilmiştir.Bulgular: Çalışmaya 5 aylık süreçte takipleri yapılan 230’u erkek, 231’i kız toplam 461 çocuğun izlem verileri dâhil edildi. Çalışmaya dâhil edilen çocuk ve gençlerin yaş ortalaması 11,87±3,33 olarak hesaplandı. Yaşa ve cinsiyete göre vücut kitle indeksi hesaplandığında %48,15’inin (n:222) normal değerler haricinde olduğu tespit edildi. Öğrencilerin %27,33’ünde (n:126) diş çürüğü, %13,66’sında (n:63) görme bozukluğu, %10,19’unda (n:47) obezite, %4,72’sinde (n:22) hiperlipidemi ve %3,25’inde (n:15) anemi tespit edildi. Tansiyonu ölçülen 4 çocukta pre-hipertansiyon, 2 çocukta Evre 1 hipertansiyon tespit edilirken işitme testi sonucu 1 öğrencide çift taraflı işitme kaybı, 2 öğrencide tek taraflı işitme kaybı olduğu görüldü.Sonuç: Çalışmanın yapıldığı aile hekimliği biriminde geçmiş dönemlere kıyasla ilgili programın da katkısı ile okul çağı çocuk/genç izlem sayısının 5 aylık süreçte %3’ten %45’e yükseldiği görüldü. Bu tür protokollerin uygulanması ile okul çağı çocuk/genç izlem sayıları artacak ve büyüme ve gelişme anormallikleri, hipertansiyon, dislipidemi, görme ve işitme bozuklukları ile ağız diş sağlığı problemleri gibi birçok hastalığa erken tanının koyulması ve/veya risk yönetimi ile önlenmesi çalışmamızda olduğu gibi sağlanabilecektir.
Abstract
Objectives: Monitoring the growth and development of children is an important issue. As a matter of fact, infamily health centers, it is stated in the relevant protocols and regulations that the students should be examinedperiodically by their family physicians every year. In this study, the results of school-age follow-ups in a familymedicine unit were examined. Materials and Methods: Our study is a retrospective descriptive observational study. The follow-up data ofall students who applied to a family medicine unit in Ankara between January 1- June 1, 2019, by referring fromschools within the scope of the "Program for the Protection and Improvement of Health at School" werecompiled and evaluated through the relevant family medicine information system. Results: 461 children were included in the study (230 of whom were boys and the mean of age was11.87±3.33). The body mass index of children was found out of normal (48.15%(n:222)). Dental caries in27.33%(n:126) of the students, visual impairment in 13.66%(n:63), obesity in 10.19%(n:47), hyperlipidemiain 4.72%(n:22) and anemia in 3.25%(n:15) were detected. Prehypertension was detected in 4 children whoseblood pressure was measured, stage 1 hypertension was detected in 2 children while hearing test result wasfound to have hearing loss in 3 students. Conclusion: In the family medicine unit where the study was conducted, it was observed that the number ofschool-age follow-up of children increased from 3% to 45% in 5 months with the contribution of the relevantprogram compared to previous periods. By applying such protocols, the number of school-age follow-ups willincrease and many diseases such as growth and development abnormalities, hypertension, dyslipidemia, visualand hearing disorders, and oral and dental health problems will be diagnosed early and/or prevented by riskmanagement like in our study.
