ORTOPEDİ HASTALARINDA AKUT OPİOİD TOLERANSI GELİŞİMİ AÇISINDAN REMİFENTANİL VE FENTANİLİN KARŞILAŞTIRILMASI

Özet

AMAÇ: Intraoperatif dönemde kullanılan opioidler, postoperatif dönemde opioid toleransına neden olabilir. Tolerans oluşumunda kullanılan opioidin analjezik potensi, toplam dozu ve kullanım süresi etkilidir. Çalışmamızda intraoperatif standart dozda kullanılan fentanil ve remifentanilin postoperatif opioid toleransı gelişimdeki etkilerinin karşılaştırılması amaçlanmıştır. GEREÇ VE YÖNTEMLER: Çalışmaya majör ortopedik cerrahi geçirecek ASA I-III, 18-70 yaş arası, 34 hasta dahil edildi. Hastalar randomize olarak 3 gruba ayrıldı. Tüm hastalara thiopental sodyum ve vekuronyum bromid ile indüksiyon yapıldı. Kontrol grubuna herhangi bir opioid verilmedi (Grup K). Grup R'ye 0.1 µg.kg.-1dk-1 i.v. remifentanil infüzyonu, Grup F'ye 1 µg.kg.-1 i.v. fentanil yapıldı. Hastaların otonomik cevaplarına göre, Grup R'de remifentanil infüzyonuna, 0.05 µg.kg.-1dk-1 doz artırımlarıyla, maksimum doz 2 µg.kg.-1dk-1 olacak şekilde devam edildi. Grup F'de fentanil ilk dozunda tekrarlandı. Grup R'de remifentanil infüzyonu cilt kapanana kadar devam ederken Grup F'ye son 30 dakikada ek fentanil yapılmadı. Her üç gruptaki hastalara cerrahi bitiminden 30 dakika önce, 0.15 mg.kg-1 i.v. morfin verildi. Uyandırma odasında hastalara 5 mg i.v. morfin sonrası i.v. hasta kontrollü analjezi (HKA) cihazı bağlanarak, hastalar 24 saat izlendi. Hastaların postoperatif ilk morfin gereksinim zamanı, ağrı ve sedasyon skorları, kullandıkları toplam morfin miktarı kaydedildi.BULGULAR: Üç grubun ilk morfin gereksinim zamanları değerlendirildiğinde, F ve R grubunun ilk morfin gereksinim zamanları kontrol grubundan anlamlı olarak kısa iken, R grubunun ilk morfin gereksinim zamanı da F grubundan anlamlı olarak kısa idi (p

Abstract

OBJECTIVE: Opioids used in intraoperative period may cause opioid tolerance in postoperative period. In development of tolerance, analgesic potency, total dose and duration of use of opioids are effective. In this study we aimed to compare the effects of standart doses of intraoperative fentanyl and remifentanyl on postoperative opioid tolerance development.MATERIAL AND METHODS: Thirty four patients, ASA I-III, aged between 18-70 which will undergo major orthopedic surgery were enrolled to the study. Patients were randomized into 3 groups. Anaesthesia was inducted with thiopenthal sodium and vecuronium bromide in all patients. In control group (Group K) opiods were not used. In Group R and Group F, 0.1 µg.kg.1min.-1 i.v. remifentanyl infusion and 1 µg.kg.-1 i.v fentanyl were given, respectively. According to autonomic responses of the patients, remifentanyl infusion was increased by 0.05 µg.kg.-1min.-1 increments to a maximum of 2 µg.kg.-1min-1 in Group R, fentanyl was repeated with 1 µg.kg.-1 in Group F. Remifentanyl infusion was continued until skin closure. Fentanyl was not repeated during the last 30 minutes of the operation. All patients in all three groups received 0.15 mg.kg-1 i.v. morphine 30 minutes before the end of surgery. In recovery room all patients received additional 5 mg i.v. morphine and patient controlled analgesia machines are connected to venous lines of all patients. Patients are followed for 24 hours postoperatively. Patients' postoperative first morphine requirement times, pain and sedation scores, and total morpine consumptions are registered.RESULTS: When the first morphine requirement times in three groups were evaluated, it was seen that patients in Groups F and R required morphine significantly earlier than in control group.
